The Intermediate Care Facility for the Individuals with Intellectual Disabilities (ICF/IID) Program enforces rules and regulations for over 500 facilities. The majority of these facilities are four to eight-bed group homes. Staff conducts certification surveys and investigates complaints. The surveys focus on observations of staff and/or consumer interaction and interviews with consumers regarding their participation and choice of services in order to determine the outcomes of active treatment.

Report a complaint regarding a health care facility
Any individual may submit a complaint to the IDOH about the care provided at any licensed or certified Indiana health care facility or entity. IDOH investigates all complaints. Report an incident regarding a health care facility
State or federal regulations require health facilities or entities to report certain types of “incidents” to the IDOH. An Incident report form is provided for health care facilities to report incidents to the IDOH.
Report a reasonable suspicion of a crime against a long term care resident
Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long-term care resident to the IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include a nursing home, hospice, or intermediate care facility for individuals with intellectual disabilities (ICF-IID).
