About the Complaint and Incident Reporting System

Program Guidance

The Indiana Department of Health (IDOH) is responsible for managing events reported by staff and personnel of comprehensive and residential care facilities. These events ("incidents") are reported through the IDOH Gateway (Incident Reporting System).

Report a Complaint

Anyone may submit a complaint to IDOH about care provided at any licensed or certified Indiana healthcare facility or entity. IDOH investigates all complaints.

This report link is to a page that provides information about the complaint process and how to report a complaint. A complaint report form is provided for individuals to assist in the reporting of care issues.

Report a Reasonable Suspicion of a Crime

Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long-term care resident to IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include nursing homes, hospice facilities, or intermediate care facilities for individuals with intellectual disabilities.

    Long-Term Care Abuse and Incident Reporting

    IDOH's Long-Term Care Abuse and Incident Reporting policy (IDOH-CSHR-LTC-002) provides guidance for reporting clarity and regulation for comprehensive and residential care facilities, including types of incidents to report, timeline for reporting, and information to include in each report. The initial incident report and follow-up report may be submitted together if all necessary information has been obtained within the timeframe for initial reporting.

    Information for an evacuation or event involving the emergency management agency must be reported by phone immediately to 317-464-0038.

  • Incident Reporting when Online System is Unavailable

    Incident Reporting when the Online System is Down

    Email

    1. Complete the Incident Reporting Form
    2. Email the completed form to Incidents@health.in.gov
    3. Within 24 hours of the online Incident Reporting System resuming accessibility, report the incident online and attach the incident report form (from #1) to the report in the Gateway.

    Voicemail

    1. Incident reports are accepted via voicemail only when the Gateway is inaccessible and email is not functioning.
      1. Please call 1-800-246-8909 and include the following:
        1. Name of Reporter
        2. Title of Reporter
        3. Name of Facility
        4. Address of Facility
        5. Type of Incident
        6. Description of Injury
        7. Name(s) of Resident(s)
        8. Name(s) of Staff Involved
        9. Details of Immediate Action
