Radiology, Weights, and Measures

The Division of Radiology, Weights, and Measures provides guidance and assistance to Hoosiers and healthcare providers regarding radiation safety and protection.

The Division of Radiology, Weights, and Measures supports the:

  • registration and compliance of radiation machines; and
  • licensing of radiology professionals, such as radiologic technologists, limited radiographers, nuclear medicine technologists, and radiation therapists.

Radiology Licensure

IDOH issues radiology permits, licenses, and certificates for eight categories.

Reciprocity

The State of Indiana does not have reciprocity with other states; this means that a permit, license, or certificate from another state is not accepted within the State of Indiana.

Requirements for Permits, Training, and Licenses/Certificates

Permits are required to complete clinical training in Indiana and are not licenses. Permits are issued to individuals when they enroll in an approved radiography education program. Upon completion of an approved education program, clinical training (with an active permit in Indiana), and passing an approved exam, an application for a license/certificate may be submitted.

Licensure Information

Initial License Application

Initial Application (Online/Mail-In)

Users may apply online using the Indiana Licensing Enterprise or by mail using this mailing address.

Online and mail-in applications are available for first-time applicants for the following licenses/certificates:

  • Radiologic Technologist
  • Radiation Therapist
  • Nuclear Medicine Technologist
  • Cardiac Catheterization Radiographer*
  • Chest Radiographer*
  • Chiropractic Radiographer*
  • Podiatric Radiographer*

Applications may be submitted by mail for a dental radiographer license after completing requirements.

Instructions Specific to Online Applications

Users may apply online using the Indiana Licensing Enterprise.

  1. Login or create credentials using the Register for an Access Indiana Account option.
  2. On the left-hand side of the page, select Initial Application under Menu.
  3. The Application for Licensure page will load.
  4. Under Profession, select Radiology.
  5. For License Type, make the applicable selection.
  6. Select Application for Obtained by Method.

Instructions Specific to Mail-In Applications

Please complete, print, and mail the license application and a check/money order for $60.00 to the address on the license application. The documents required* with applications may be mailed together with the completed application and paid to the address on the license application.

Instructions Specific to First-Time Dental Radiographer License Applications

After meeting education, clinical training (with permit), and exam requirements, applications may be submitted by mail. Please complete, print, and mail the license application and a check/money order for $60.00 to the address on the license application. IDOH cannot process licenses until all required documents have been received. Graduates of the Commission on Dental Accreditation (CODA) are not required to provide a proficiency form. For questions about required documents, please contact the program director of the approved radiography education program.

Documentation is required for a first-time license/certificate application.
A digital scan of the following documents is required for an application:

  • Clinical training form completed by the director of the approved radiography education program or the licensed practitioner who supervised the clinical training while the provisional permit was active.
  • Education diploma/certificate of completion from the approved radiography education program.
  • Proof of passing an approved exam.

License/Certificate Renewal

Users may renew licenses/certificates online using the Indiana Licensing Enterprise.

  1. Login using the Register for an Access Indiana Account option.
  2. On the left-hand side of the page, select Renew License under Menu.

If you are unable to renew your license online, you may send your renewal form and payment to the address on the renewal form.

Please send an email to Radiology@health.in.gov if you have two licenses to renew. After IDOH waives the renewal fee for one license, you may renew both licenses online.

Verifying Active Licenses/Permits Online

To verify an active license or permit, please access Search and Verify.

To request the verification of a license for a travel technologist (e.g., a form is required by another state), please send the verification form from the state to Radiology@health.in.gov or fax it to 317-233-7154. The top portion is completed by the technologist and the bottom portion is completed by IDOH.

Address/Name Changes to License/Permit

Address Change

Verifying that IDOH has accurate records will support the timely reminder notification and delivery of licenses and permits. If your address has changed, please update it by using one of the methods below.

There is not a fee for address changes.

Name Change

Verifying that IDOH has accurate records will support the timely reminder notification and delivery of licenses and permits. If your name has changed, please update it by using one of the methods below. After IDOH has received the necessary information, the database will be updated and a new license will be issued.

Requirements

There is a $20.00 fee for name changes.

Please include the following information with change requests that are submitted via email:

    • Specifically, proof of name change (e.g., copy of marriage license, pages of divorce decree, legal name change document, etc.).

File a Complaint

Consumers may file a complaint about users operating a radiation-producing machine or a medical practitioner for their employment of a radiographer or the use of their radiation-producing machine(s) by emailing Radiology@health.in.gov or mailing the address below.
