The Division of Home and Community-Based Care is responsible for licensing and certification programs for home health agencies, home health aides (HHAs), personal care services agencies (PSAs), mobile out-of-state healthcare entities, hospice agencies, and end-stage renal disease facilities (ESRD).

Licensing and Certification Programs
Report a Complaint
Anyone may submit a complaint to IDOH about care provided at any licensed or certified Indiana healthcare facility or entity. IDOH is available to receive complaints against nursing homes, hospitals, home health agencies, hospice agencies, personal service agencies, and intermediate care facilities for persons with intellectual disabilities (ICF-IID; also called group homes). IDOH investigates all eligible complaints.

Report an Incident
State or federal regulations require health facilities or entities to report certain types of “incidents” to IDOH. This report link is to a page that provides information about the incident reporting process and how a healthcare facility or entity reports incidents.
