About the Healthcare Facility Information Center

The Consumer Services and Healthcare Regulation (CSHCR) Commission at the Indiana Department of Health (IDOH) promotes healthcare quality by coordinating the licensure and certification of health facilities, clinics, centers, and providers; supporting quality initiatives of patient care; ensuring radiation safety among clinical staff; and confirming state standards for weights, measures, and metrology.

The Healthcare Facility Information Center is an initiative of CSHCR and intends to provide clinical staff with increased access to program policies for the licensure and certification of healthcare facilities.

Report a Reasonable Suspicion of a Crime

Report a Reasonable Suspicion of a Crime

Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long-term care resident to IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include nursing homes, hospice facilities, or intermediate care facilities for individuals with intellectual disabilities.

This report link is to a page that provides information for long-term care providers about the crime reporting requirements and how to report a reasonable suspicion of a crime against a resident. A crime report form is provided for individuals to report suspected crimes to IDOH.
