The Division of Acute & Continuing Care includes licensing and certification programs for more than two dozen types of healthcare providers in Indiana, including hospitals, ambulatory surgery centers, and abortion clinics. Some of these programs have individual home pages that are linked below.

Emergency Order Reissuing Temporary Blanket Waivers for Hospitals and Ambulatory Outpatient Surgical Centers
NOTICE to All Hospitals & Ambulatory Surgical Centers: IDOH’s COVID-19 Regulatory Blanket Waivers expired on July 1, 2022. Facilities may request an individual waiver to continue temporary changes to physical plant requirements beyond July 1. Individual waiver requests may be submitted to IDOH at any time. If a facility intends to make any physical plant changes permanent, they must submit a plan review application and the plans must be received by IDOH Health Care Engineering within 60 days of the application being submitted. Facilities do not need to submit a request for a temporary waiver if the plan review application has been submitted and may continue to operate until the plan review process is finalized.


Fetal Remains Disposition Letter – December 2022

IDOH Guidance for Implementation of Fetal Disposition Requirements from House Enrolled Act (HEA) 1337
As a result of a recent U.S. Supreme Court decision, an injunction preventing the enforcement of certain provisions of HEA 1337 from 2016 was lifted September 3, 2019 and these provisions are now enforceable. This law updated the requirements for the disposition of miscarried remains/aborted remains (“fetal remains”) in Indiana.

Report a complaint regarding a health care facility

Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility

The Incident Report Form is for health care facilities to notify the Indiana Department of Health of a reportable incident pursuant to the IDOH Reportable Unusual Occurrence Policy.  The Incident Report Form is also for health care facility staff (nursing homes, intermediate care facilities, and hospice agencies) to report a reasonable suspicion of a crime against a resident pursuant to Federal regulations.  This form is not to be used to file a complaint.
