Summary of Case Management Services (CMS)

The statewide contracted Case Management Services are provided by Deaf Community Services, located at 4740 Kingsway Drive, Indianapolis, IN 46205, with support from subcontractors throughout the state of Indiana that includes 92 counties.

These case managers specialize in using their ASL and knowledge of Deaf Culture, work closely with individuals who are:

  • Deaf
  • Deaf-Blind
  • Late Deafened
  • Hard of Hearing

The Criteria of obtaining the Case Management Services – the individual must be:

  • Reside in the State of Indiana
  • Be Deaf, Deaf-Blind, Hard of Hearing, Late Deafened
  • 16 years old or older
  • Be position that their independence is in jeopardy or chance of being institutionalized without needed supports
  • Any other appropriate services designed to assist an individual in coping with daily living activities including accommodation, community, health, home, mental health, social services, and social skills
  • Services related to securing housing or shelter, including services related to community group living, that are supportive of the purposes of the Rehabilitation Act of 1973

They assist in navigating a broad range of services, including:

  • Accessibility and accommodations
  • Community and social connections
  • Health and mental health support
  • Medical care and medical monitoring
  • Housing and home stability
  • Social services and skill-building

The service aims to help individuals enhance their functional level ability and independence.  Support may be offered on a short-term, long-term, or one-time basis depending on their intake assessment and their requested assistance.
