HCBS Medicaid Waiver Waiting List Information

The PathWays Medicaid Waiver for individuals aged 60 and older and the Health and Wellness Medicaid Waiver for individuals 59 and younger provide home and community-based services to eligible Hoosiers who meet nursing facility level of care and Medicaid eligibility requirements.

As approved by the federal government, 55,969 total slots are available between the two waivers during the timeframe July 1, 2025 through June 30, 2026. The Family and Social Services Administration reached its federally approved capacity in April 2024 and implemented a waiting list.

FSSA will begin inviting individuals in July 2025 from the Health and Wellness and PathWays Medicaid Waiver waiting lists each month as long as capacity remains.

FSSA must hold slots in reserve during the waiver year for individuals to access the waiver as based on the approved waiver application. The reserved slots will reduce throughout the waiver year as individuals who meet the criteria are given a slot to move forward. A table below each waiver information identifies the categories and slots held in reserve.

As capacity remains on the waiver, Individuals are invited each month from the waiting list in the following order:

  1. Those transitioning from Nursing Facilities, CHOICE, and Discharge from Hospital receive first priority of targeted waiver invitations each month, per the waivers approved by the Centers for Medicare and Medicaid Services, regardless of the date they were assessed for nursing facility level of care;
  2. Individuals on the waiting list without a priority status receive the remaining targeted waiver invitations based on the original level of care assessment date.

Individuals invited to proceed from the waiting list will receive a letter, samples below, with further details on next steps.

*Effective July 1, 2025, the Health & Wellness (H&W) and PathWays Medicaid waivers have a designated reserved waiver capacity for individuals residing in assisted living who meet the defined criteria. For more information, please review the procedure and referral form.

Health and Wellness Medicaid Waiver

  • During the timeframe July 1, 2024 through June 30, 2025
    • 16,127 slots were allotted in the Health and Wellness Medicaid Waiver
    • 15,066 slots were actively filled
    • 3,717 invitations were issued
    • 1,060 invitations remain open

    To join the Medicaid Waiver, you must have both Medicaid eligibility, financial eligibility, which means full Medicaid coverage with an Aged, Blind, or Disabled benefit aid category, and functional eligibility, Nursing Facility Level of Care.

    In preparation for your conversation with your AAA and waiver invitation, please see the ‘Apply for Medicaid Guidance,’ which includes a list of documents you may need. This document is included with your invitation letter and is also held by your AAA.

    Please note that the Medicaid application process may go more smoothly if you get help from the AAA. They have years of experience, are well versed in the application process, and offer their services at no cost to the individual.

  • What is my spot on the waiting list? When will I be invited from the waiting list?

    Individuals on the PathWays Medicaid Waiver and Health and Wellness Medicaid Waiver waiting lists do not have a static place in line. The waiting list is fluid, as individuals are added on a daily basis with varying levels of care criteria and dates that impact how individuals are invited. Please see the text above this FAQ for information on how invitations are sent, first for approved priority categories and then based on the NFLOC assessment date.

    In order to increase transparency and help individuals, families, and other stakeholders get more information about the latest invitations, FSSA has added the latest date to be invited to the Waiting List Webpage, linked here.

    To confirm that you have been added to the waiting list, you may visit the online dashboard. Please note it will take at least five business days for your name to be added to the waiting list. A guide to accessing and using this resource is available here. If you have additional questions, please reach out to your AAA. Please note that your AAA will not be able to tell you about your place on the waiting list, nor will FSSA, but can provide information regarding your level of care date or priority category status.
