Reporting a Worker Injury, Illness, or Fatality in Indiana
The federal Occupational Safety and Health Administration (OSHA) updated its reporting requirements in 2015 to include certain work-related injuries.

Work-related Amputations, In-Patient Hospitalizations, and Loss of Eye
Within 24 hours, employers must report all work-related amputations (with or without bone loss), the in-patient hospitalization of one or more employees, and workplace incidents resulting in the loss of an eye to IOSHA. Employers may report such incidents using the online Serious Event Reporting Form.
Work-related Fatalities
Within 8 hours, employers must report all work-related fatalities to IOSHA. Work-related fatalities must be reported by phone to IOSHA.
Reporting a Fatality During Normal Business Hours: To report a worker fatality during normal business hours – 8 a.m. – 4:30 p.m. EST., Monday-Friday (excluding State holidays), please call IOSHA Customer Service at (317) 232-2693.
Reporting a Fatality After Normal Business or on a State Holiday: To report a worker fatality after normal business hours or on a State holiday, please contact the OSHA After-Hours Hotline at 1-800-321-OSHA or 1-800-321-6742 and speak with the Duty Officer.
Please be prepared to provide the following information in your report:
- Establishment/Company Name;
- Establishment/Company Address;
- Name, phone number, and email address (if available) of contact person;
- Address of where the incident occurred;
- Date and approximate time of the incident;
- Name(s) of fatally injured employee(s); and
- Brief description of the incident.
