Connect to Cure is a linkage to care program for Hoosiers living with hepatitis C. Connect to Cure is a program developed by community partners based on initial work funded and overseen by the Indiana Department of Health. Today, there are state funded Connect to Cure sites, as well as community partner funded sites. The primary goal of Connect to Cure is to increase the number of persons in Indiana who are aware of their HCV status and are linked to care, by helping those who are living with hepatitis C find and establish care; ensure continuity of care; promote self-sufficiency through customized education; coordination of services and empowerment of the individual. Eligibility
- Has tested positive for the hepatitis C virus (regardless of symptoms or date of diagnosis)
- Resides in Indiana
How the Program Works

Care Coordinators can assist with:
- Scheduling medical appointments
- Insurance enrollment
- Transportation
- Housing
- And more
