STI Surveillance

In Indiana, cases of chlamydia, gonorrhea, syphilis, and congenital syphilis are required to be reported to the health department in your area within one business day of diagnosis; healthcare providers should not report cases of herpes or trichomoniasis. Cases can be reported via the Indiana Confidential Sexually Transmitted Disease (STD) Report State Form 56459 by fax, or online via the REDCap Survey links below. Diseases should be reported separately in the appropriate survey (e.g., Gonorrhea cases should be reported in the Gonorrhea survey and NOT in the Chlamydia or Syphilis survey). Fax numbers for each STI district (where your local health department or affiliated community-based agencies are located) are included on the STI DIS Contact Map (link below).

Data Dashboards

The STI Morbidity Dashboard includes Indiana STI data from 2019-2022. Data can be broken down by county, STI districts, and the following demographic information (race, sex, and age group).

The 2022 Chlamydia, Gonorrhea, and Syphilis maps include national averages taken from the CDC's 2022 STI Surveillance Report. Indiana data were taken from the state's NEDSS-based Surveillance System (NBS).

2022 Indiana STI Data Trends

The 2022 STI Surveillance Spotlight Report and data trend pdfs are now available. The data trends report includes 2018-2022 rates for chlamydia, gonorrhea, and adult (all stages) syphilis. It also includes Indiana's congenital syphilis morbidity from 2018-2022. The pdf is available for download here.
