Training Provider Application - Revis. 4/24
As of July 1, 2021, the fees associated with Training Provider Applications will be as follows:
Provider Application (New and/or Renewal – 2yr. certification) - $500 - To apply you must fill out a Training Provider Application and submit, by mail, with all documents required. This authority, if approved, will require re-certification every two years.
