The IDOH team is currently piloting the implementation of the (PPDT) safety bundle from AIM at select hospitals. Currently, 10 hospitals are participating in the pilot program. Sites are primarily responsible for recording bundle metrics within the AIM Data Center and sharing with the IDOH team progress made, lessons learned, and any challenges they've encountered while implementing the PPDT safety bundle.
Additionally, hospital sites are required to hire a Maternal Health Advocate (MHA) as part of the program. The MHA's overarching purpose is to provide social support to mothers throughout the perinatal period. The individual in this role is someone from the community whose responsibilities would include:
- Screening patients for perinatal and postpartum health conditions then elevate those concerns if additional medical or community-based support is needed. Read
- Connecting mothers to access appropriate local resources such as transportation, housing stability, food access, and other barriers impacting care MHAs provide
- Advocating for patients by reinforcing a patient's voice, supporting communication with care teams, and helping patients navigate services, referrals, and follow-up care
