Home Visiting Referral System

Indiana’s home visiting referral system was created to connect pregnant women with home visiting programs in their local community. To make sure each woman is referred to a program that can meet her needs, the team with Indiana’s home visiting referral system worked with local home visiting programs to develop county-specific referral plans.

Today, the referral system is operated through Moms Helpline.

You can make a referral by completing a simple form.

If your organization is interested in receiving referrals from the referral system, the first step is to complete a short application form and send it to HVMomsHelpline@health.in.gov. The application will reference:

Once you have established a partner agreement, you will receive your referrals through our Closed Loop Referral System. We have developed resources to guide you through this process.
