How to Become a Provider
Step 1) Apply for DMHA Certification (FSSA: DMHA: Quality Assurance / Quality Improvement)
Step 2) Complete the below forms and send to: recovery.works@fssa.in.gov
- Copy of DMHA Certificate
- Recovery Works Agency Application
- W9 form
- Direct Deposit form
- WITS/JIRA form
- Agency Profile Packet
- Contact form
- Copy of Voided Check
- DARMHA Registration Agreement
- DARMHA New User Form
Step 3) Send forms to recovery.works@fssa.in.gov During this step, Recovery Works is working to get your agency set-up in the State of Indiana Systems. This step can take up to 30 days for completion. Recovery Works will reach out to the agency at the completion of this step.
- Send the documents from Step 2 and a voided check to recovery.works@fssa.in.gov
- Please note, the application process may take 30-45 days to complete. During this phase, Recovery Works is running the application through the State of Indiana Clearance Process, registering the agency with the State of Indiana Auditor’s Office as well as Recovery Works systems. Recovery Works will reach out to the applicant when ready to move to the next phase. The next phase includes signing the provider Agency Agreement and receiving the Recovery Works Welcome Letter.
Step 4) Application Checklist
