Residential Care Provider Certification
Residential care provider means an organization that the division has certified as fulfilling the statutory and regulatory requirements to be a residential care provider. A residential care provider must apply separately for certification or licensure of the specific facility they tend to operate. A provider must be compliant with 440 IAC 6.
If your residential setting does not provide SUD treatment and your entity is not already certified as a Community Mental Health Center, you need to be certified as a Residential Care Provider to operate a residential setting under 440 IAC 7.5.
Completing the application in the DMHA Provider Portal, you will be asked to provide the following information via rich text:
- Policies and procedures for applications in DMHA Provider Portal (Note: these are required to be entered as rich text
- Confidentiality according to IC 16-39
- Services to be provided (440 IAC 6-2-4(a)(2))
- Resident’s rights according to IC 12-27
- Documents that should be uploaded in the application in the DMHA Provider Portal:
- A description of the organizational structure and mission statement of the applicant.
- A list of governing board members (at least five (5) members)
- At least one member is a primary
- At least one member is a secondary consumer
- At least one member has a current license as a physician or health services professional (HSPP) – provider must submit copies of license/license number’s
- A list of names and positions of executive staff.
- Copy of CEO’s resume
- CEO must have at least a Master’s degree
- CEO should have demonstrated experience in managing mental health care or a related field
- Proof of general liability insurance coverage in the minimum amount of five hundred thousand dollars ($500,000) for bodily injury and property damage.
- Copy of client’s rights document that is given to consumers
- The most recent audit of the residential care provider, which shall be prepared by an independent certified public accountant.
- Copies of the current professional health provider license of a board member and the professional services director.
