Eyelash Extension Specialist Training Program Recognition

Per Indiana State Rule 410 IAC 1-8-12, any individual(s) who are not licensed estheticians or licensed cosmetologists, and who wish to practice the application of eyelash extensions for the public, must obtain a certification from a program recognized by the Indiana Department of Health (IDOH). IDOH will review Eyelash Extension Specialist Training Programs to determine if they meet the State requirement for recognition. IDOH does not provide, nor maintain record of, certification to individual students or people.

Submission Application for Certification Programs to be Recognized by Indiana Department of Health

As of May 27, 2024, IDOH will no longer review program applications submitted by email or fax. If you would like your Eyelash Extension Specialist Training Program to be considered for IDOH recognition, follow the link below to complete the online application. You will be able to attach supportive documentation within the online application.

If you have any trouble accessing the online application, please email EyelashCert@health.in.gov. Applicants will be notified of their recognition or denial via the email address provided with their application. You can expect to hear a response back from IDOH within 7-14 business days.

Complaints

IDOH will review complaints of allegations related to Eyelash Extension Specialist Training Programs being out of compliance with the State rules. IDOH will not review complaints of allegations related to individuals. If the individual of concern is a licensed cosmetologist or licensed esthetician, please contact the Indiana Professional Licensing Agency . If the individual is not licensed, but may hold a professional certification, please contact the office of the  Indiana Attorney General . If the concern is related to the establishment in which eyelash extension services are being provided, please contact the applicable county health department. Complaints related to training programs recognized by IDOH may be submitted via email to EyelashCert@health.in.gov.
