Mobile Response Operations

Request Mobile Services

DEP has 10 mobile units managed by the Mobile Response Operation section. These units deploy upon request to provide public health services including, but not limited to, school health screenings, routine immunizations, COVID-19 vaccination and testing, HIV/STI testing, and phlebotomy training.

Any school nurse, administrator or school liaison who would like to request an IDOH mobile hearing screening unit hearing screening should contact the school liaison in their county or Lorrie Ramsey, chief nurse consultant (CNC), at backtoschool@health.in.gov or submit the request form below.

  • How to Request a Mobile Clinic

    van exterior If you are interested in hosting an onsite clinic but have questions, you should contact the appropriate IDOH Mobile Operations staff for your region. Please reach out at least 3 to 4 weeks ahead of the event.

  • Vaccine Clinic Checklist

        • Pfizer COVID Vaccine (Age 5 and up)
        • Pfizer COVID Bivalent Boosters (Age 5 and up)

    • A registration link. We ask that you advertise the clinic and encourage pre-registration. Paper forms can be made available in both English and Spanish.

    School Hearing Screening:


    The Indiana Department of Health (IDOH) has launched an initiative to make mobile units available to complete hearing screening at schools. The units are available for students in grades 1, 4, 7 and 10, as well as any student transferring in or suspected of having a hearing impairment, as required by Indiana Code 20-34-3-14.

  • Homebound Hoosier Vaccination Clinic

    The specific goal of the Homebound Hoosier vaccination project is to administer COVID-19, Tdap, Pneumonia, and (when available) Flu vaccines to at-risk, hard to reach, or otherwise immobile or homebound individuals at no out-of-pocket cost to the recipient.

    No government issued ID or insurance is required, though insurance will be billed if available. Anyone under age 18 must have a parent or guardian with them to give consent.

  • Phlebotomy Training

    The Division of Emergency Preparedness is offering a nursing refresher of basic phlebotomy procedures with didactic and hands-on skills for local health departments and other public health stakeholders.

    Program Details:
    • This class does not provide certification.
    • Training availability is limited and will be approved on a first come, first served basis.
    • Training classes have a maximum capacity of 20 participants.
    • The requesting facility will be responsible for providing the training location and blood collection supplies to use for hands-on practice.
    • If a facility chooses to include non-nursing staff in the training, the facility should provide a trained nurse or phlebotomist on-site to oversee future procedures performed by said staff (preceptor program).
    • The training presentation will not be distributed online and is only available during the in-person training.
