Any school nurse, administrator or school liaison who would like to request an IDOH mobile hearing screening unit hearing screening should contact the school liaison in their county, Andrew Wilson or submit a request form by clicking the link below.
Request Daycare Lead Testing
Launched in May 2024, our mobile units support childcare providers who want to offer lead testing to the families of children at their site. Children younger than age 6 are most vulnerable to the lifelong, hazardous impacts of lead exposure, and testing is the only way to know for sure whether a child is being exposed to lead.
If you are a childcare operator and would like to have one of our mobile teams come to your site to conduct testing, please take a moment and fill out the form linked below.
If you have questions about lead testing before you fill out the request, feel free to reach out to Kari Horsley at 317-233-1296.
Request Oral Health Screening
Oral health provides guidance on screening patients for the purpose of identifying early signs of tooth decay and gum disease that will require the attention of a dentist in school aged children. This screening is designed to encourage caregivers to take positive action and ensure timely dental care.
- How to Request a Mobile Clinic
If you are interested in hosting an onsite clinic but have questions, you should contact the appropriate IDOH Mobile Operations staff for your region. Please reach out at least 3 to 4 weeks ahead of the event.
