Physician-owned medical practices must apply with DOR using INTIME in order to file this credit for its owners after June 30 of a calendar year but not later than June 30 of the subsequent calendar year. If you do not have an INTIME account, you will need to create one. Visit intime.dor.in.gov, select “New to INTIME? Sign up” and follow the instructions provided.
Applications must include a certified list of each physician who has an ownership interest in the income of the medical practice described in IC 6-3.1-40-6. DOR may determine more information is needed when calculating the credit.
Physicians must be in the practice of medicine and provide health care services to patients for at least six months of a given calendar year.

- How do I need to list my individual Indiana physician’s license in INTIME?
You will need to include leading zeros but do not include any letters. If you are not a licensed physician, enter “NLN.”
- How do I need to enter the owner’s percentage of income or loss from the practice?
The percentage should be stated as a percentage, rounded to the nearest one-hundredth of a percent. Guaranteed payments as part of a partnership should not be considered.
- How do I need to enter my information as a non-qualified owner?
Enter “X” if any of the following apply:
- The owner is not an individual.
- The owner is not a licensed physician in Indiana.
- The owner owns less than the following percentages in the income of the entity:
- 5% if there are 10 or fewer owners in the practice
- If there are more than 10 owners in the practice, the percentage ownership is less than 50% divided by the number of physicians owning an interest in the practice.
If the practice is owned by more than 10 owners but fewer than 10 physicians, the owner must own a minimum of 5%.
- The physician can claim the credit for the 2024 tax year (limited to primary care physicians).
- The physician can claim the credit as a result of ownership in another entity.
- The physician is employed by a hospital system.
