This table is the IDHS schedule of fines and civil penalties that apply to violations of laws, rules and requirements of federal programs administered by the agency.

IC 10-19-12-18(a)	Radiation violations	Up to $10,000 per violation per day	Fines will not be implemented until rules are drafted.
IC 16-31-3-14(b)	Violations by EMS certificate/license holders	Up to $500 per violation per day	For acts of renewal dishonesty and late applications by provider organizations, fines are applied as identified in Note 1 and Note 2. For all other instances, fines are stayed until rules are drafted.
IC 16-31-3-17	Practicing EMS without certificate/license	Up to $500 per occurrence	A fine of $500 is issued per occurrence.

IC 22-15-4-7(c)	Manufactured home violations	Up to $1,000 per violation	Not applied. Enforcement applied in accordance with the procedure identified for IC 22-12-7-7.
IC 22-15-5-16	Regulated lift device practitioner violations	Up to $1,000 per violation	Not applied. Enforcement applied in accordance with the procedure identified for IC 22-12-7-7.
