Processing Fee
Customers of this site will be charged a non-refundable processing fee of $2.50 of 3%, whichever is greater, by LexisNexis VitalChek Network Inc.
Charge Information
If an application is required for the payment category you need, it must be received by the Floyd County Health Department - Environmental Health office 5 or more days prior to review to avoid delays or possible cancellation of your order.
You are authorizing the Floyd County Health Department to charge up to $400.00 depending on the payment category you select. If the funds are not available when the card is authorized your order may be cancelled. Once the Floyd County Health Department office has confirmed receipt of the required applications (if applicable) and authorized your payment, you will receive an emailed receipt at the email address provided on the order. By submitting this request, you are authorizing a charge to your credit card for the amount entered plus the LexisNexis VitalChek Network Inc. processing fee.
