To submit a violation complaint, click here to contact your local Excise District and contact the district in which the business is physically located.
Final Inspection Request by County
If needing to schedule a floorplan inspection for a facility alcohol permit that is either:
(1) an Active status and expired over 30 days (NOTE: If no floorplan changes, a floorplan affidavit can be completed instead. A link is below.)
