Submit a Violation Complaint
To submit a violation complaint, click here to contact your local Excise District and select the district in which the business is physically located, based upon the map of Indiana at the top of this page. Click on the particular district and then select the email option for that district.
Final Inspection Request by County
If needing to schedule a floorplan inspection for a facility alcohol permit that is either:
(1) an Active status and expired over 30 days (NOTE: If no floorplan changes, a floorplan affidavit can be completed instead. A link is below.)
