Pursuant to Indiana Code 24-4-25.5, this form needs to be completed and submitted by an Indiana media entity with a foreign ownership that is equal or greater than five percent (5%) of the fair marked value, market capitalization, or total voting rights of the Indiana media entity; or if an Indiana media entity has received at least ten thousand dollars ($10,000) in funding, in aggregate, from one (1) or more foreign entities. All applicable definitions may be found in Indiana Code 24-4-25.5.
This form must be completed prior to December 31, 2025, and before December 31 each year thereafter.
