An Indiana Controlled Substance Registration ("CSR") may be issued for one of the following practitioners:
Announcements
On July, 1, 2025, the requirement that an applicant for initial controlled substance registration or renewal of an existing registration complete two (2) hours of continuing education on the topic of opioid prescribing and opioid abuse expired. See Indiana Code 35-48-3-3.5.
Applicants who submit applications for initial registration or renewal of registration after July 1, 2025, will not be required to provide proof of completion of opioid continuing education.
Controlled Substance Registration Information
- Application Instructions
Applicants must have an Active Primary Indiana Practitioner License before they can obtain a CSR.
- Application fee: $60.00 by credit or debit card. Additional online processing fees apply. All application fees are nonrefundable.
- Practice Location: Applicants must use an Indiana practice address when applying for a CSR. A CSR will only be issued to a street address; post office boxes are not acceptable. An application with an incomplete or out of state address will be rejected. This practice address should match the address on your DEA Registration.
- Controlled Substance Schedules to Request: Applicants may apply for authorization for Schedules II through V; a Schedule I controlled substance has no accepted medical use and are generally restricted to researchers only.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. You must also submit documentation regarding the incident(s) in question, this documentation includes the following: court documentation that indicates charges, plea, sentencing requirements, and, if applicable, verification of completion of sentencing requirements; and any orders indicating action filed or taken against their DEA registration.
DEA Registration: The practitioner must hold an Indiana CSR and a federal Drug Enforcement Agency ("DEA") registration or is exempted from registration pursuant to 856 IAC 2-3-5(b) or 856 IAC 2-3-6 in order to prescribe, administer, or dispense controlled substances in the State of Indiana.
- Renewal Instructions
In order to renew your CSR, your Primary Indiana Practitioner License must be Active. Each registrant will receive renewal information for their CSR at the same time that he or she receives renewal information for his or her professional license with the Professional Licensing Agency. The Renewal Notice is sent no later than 90 days prior to the expiration.
- Application fee: $60.00 by credit or debit card. Additional online processing fees apply. All fees are nonrefundable or nontransferable.
- Practice Location: At the time of renewal, you may only review your Practice Location Address
- Change Practice Location Address: Controlled substances registrations are issued for a particular purpose at a specified location. Any change of address for practitioners must be reported to the appropriate licensing board. The request for change of location may be submitted in your MyLicense One account using the 'Upload Additional Documentation' Action Item on your linked CSR registration. Your request must include your name, license number, and both your old and new addresses. Once your Practice Location Address has been updated, you may log back into your account online and print a new license copy with the updated address.
- If you have changed your practice location and your existing CSR has expired, you may apply for a new CSR instead.

Reinstatement
If your CSR has been expired for 3+ years, you must reinstate the existing CSR if the practice location is the same.

- Location and Address Information
Practitioners must hold a CSR in order to prescribe, administer, store, dispense or otherwise handle controlled substances in the State of Indiana. A separate registration is required for each location where a practitioner administers, stores or dispenses controlled substances, unless the practitioner does so under a facility's CSR in accordance with 856 IAC 2.
CSR-Veterinarians Only: Senate Enrolled Act 104 amends IC 35-48-3-3 to provide that a person licensed to practice veterinary medicine in Indiana holding an Indiana controlled substance registration is not required to obtain a separate registration to possess or dispense controlled substances in the usual course of veterinary practice at a site, so long as the site is not a principal place of business or professional practice.
To change the location for an existing CSR Practitioner license, you may now submit these requests in your MyLicense One account using the Change CSR Location Action item. After submitting the request, Board staff must review your submission and if requirements are met, your CSR location address will be updated. If any issues arise, you will be sent an email potentially requesting additional information.
E-Prescribing Waiver Information
- Application Instructions
Statutorily created exemptions to the e-prescribing requirements may be found at IC 25-1-9.3-9; with authority granted to the Board of Pharmacy, in consultation with the Medical Licensing Board, to create a waiver which prescribers may apply for if they are unable to initially comply with the e-prescribing requirement.
- The Prescriber should login to, or create, their Access Indiana account at the link above.
- Once logged in, ensure your Primary License is Linked to the MyLicense One Dashboard (This must be completed first.)
- Select the Initial Application' button
- Select 'Pharmacy Board' from the list of Professions, select 'Individual', select 'ePrescribing Waiver', then select NEXT and START APPLICATION
- Complete each section of the application and ensure you push SUBMIT
NOTE: A prescriber must identify which of the categories approved by the board they are eligible for, then complete the waiver request
Recognized Exceptions
- A prescriber who demonstrates electronically transmitted prescriptions for controlled substances creates an economic hardship on behalf of the Prescriber.
- A prescriber who demonstrates electronically transmitted prescriptions for controlled substances is not possible due to technological limitations that are not reasonably within the control of the Prescriber.
- A prescriber who reasonably determines that it would be impractical for the individual involved to obtain an electronically transmitted prescription for controlled substances in a timely manner and such delay would adversely impact the individual’s medical condition.
- A prescription issued by a Prescriber under an approved research protocol.
- A prescription issued by a Prescriber for an individual who receives hospice care and that is not covered under the hospice benefit under 42 U.S. Code 1395 w-104.
- Prescriptions issued by a Prescriber for an individual who is a resident of a nursing facility, as defined in 42 U.S. Code 1396r. Waivers are not required for drug orders.
- A prescription issued by a Prescriber for an individual who is confined in a city or county jail.
- A prescription issued by a Prescriber that is a compounded prescription containing two (2) or more products.
- A prescription issued by a Prescriber that is a compounded infusion prescription containing two (2) or more products.
- Renewal Instructions
- The prescriber should log into their MyLicense One account.
- Once logged in, ensure your licenses are linked on the MyLicense One Dashboard
- Select the "Renew License" Action item for the ePrescribing Waiver
- Complete the prompts to submit the renewal of the waiver to the Board of Pharmacy, ensuring that you provide documentation as to why you still require the waiver.
If a prescriber's application for a waiver is approved by the Board, it will run concurrent with the prescriber's controlled substance registration (CSR). Therefore, a prescriber must renew their e-prescribing waiver if (1) their CSR has expired; and (2) the waiver is still needed to prescribe controlled substances which must be renewed alongside the practitioner’s.
- Verification of Waivers
If a pharmacist wants to verify if a prescriber has received a waiver from the Pharmacy Board, they can confirm utilizing PLA's Search & Verify Tool. This tool can be used to verify if the prescriber has an approved or pending application with the Board. A pharmacist can search using the waiver number, the prescriber's name, or the prescriber's CSR number.
Prescribers who have pending waiver requests before the Board are authorized by the rule to indicate that a waiver is pending on their written or call-in prescriptions so that they may continue to prescribe for patients while the Board reviews their application. Pursuant to Indiana law, the pharmacist filling the prescription shall exercise their professional judgment in the best interest of the patient's health when reviewing all prescriptions and maintains ultimate discretion in filling the prescription.

DEA
Once your Indiana Controlled Substance Registration has been issued, you may apply for the Drug Enforcement Administration (DEA) registration. You must apply for a federal Drug Enforcement Administration (DEA) registration by going to their website. If you have further questions, you may contact the DEA at (571) 362-4707.
