Surveillance and Investigations samples submitted for analyses from the Indiana State Department of Health Food Protection Division and other government agencies (Local Health Department Health) must meet ISDH laboratories, Food Microbiology requirements.

All Food / non-food products under surveillance and/or investigation can be submitted for analysis. The sample should be collected in its original packing material. If not, an appropriate sterile container should be used to collect the sample. A control sample of the identical product with the same date and manufacturer code needs to be collected with the complaint sample.
- A sample should meet all following criteria before it is accepted for analysis.
- The sample is received in a locked grey box from Food Protection.
- Sample must be submitted with a completed state collection Report with signatures.
- The chain of custody form must be completed. The form is an orange card which accompanies the sample on the out side of a locked grey box.
- Time and Date stamp each Sample Collection Report.
- State seal must be present on the sample or notated if absent.
- Sample should be in original manufacturer’s packing material or an appropriate sterile container.
- The sample must be delivered in satisfactory physical condition and without any leakage.
- The sample shall be delivered to the lab at the proper temperatures normally associated with the product.
- A control sample of the identical product with the same date and manufacturer code should be with the sample
