Speech Language Pathology & Audiology Licensing Information

Application Instructions

  • General Information

    The Fair Information Practice Act:  In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications:  If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • Speech-Language Pathology & Audiology by Examination

    Submit the following with your online application:

    1. Application Fee of $150.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Criminal Background Check.
    3. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    4. Positive Response Documentation: If you answer "Yes" to any disciplinary/background questions on the application, explain fully in a statement that includes all details. Include the violation, location, date and disposition. Submit copies of court documents for each instance to support the statement.
    5. Official Transcript: Applicants must submit a copy of an official transcript of grades from the speech-language pathology or audiology school, verifying that the applicant possesses a Master’s degree or its equivalent from an accredited academic institution in the area for which the applicant is applying for licensure. Please upload at the time of application. *Audiologist must possess a doctoral degree or its equivalent and completed a minimum of one thousand eight hundred twenty (1,820) clock hours of supervised clinical practicum, including a clinical experience that is equivalent to a minimum of twelve (12) months of fulltime supervised experience obtained during completion of the Audiology degree program.
    6. Direct Supervised Clinical Experience: Applicants must submit certification from their educational institution that the applicant has completed four hundred (400) hours of direct supervised clinical experience.
    7. Clinical Fellowship: Applicants must submit certification of completion of a clinical fellowship, which meets requirements of IC 25-35.6-1-5(5) and 880 IAC 1-1-3. Submit to the Board evidence of completion of at least nine (9) consecutive months, of full-time employment or its equivalent not to exceed a maximum period of eighteen (18) consecutive months. A clinical fellowship of fewer than fifteen (15) hours per week will not fulfill any part of the supervised experience requirement.
    8. Official ETS-Praxis Series Scores: Applicants must submit an official score report from ETS-Praxis Series, verifying a passing score on the examination. Request that your official scores be sent directly to the Indiana Professional Licensing Agency from ETS.
    9. Certificate of Clinical Competence from ASHA: Applicants may submit a current Certificate of Clinical Competence (CCC) from the American Speech-Language Hearing Association as a substitute for the following requirements: Official Transcript, Direct Supervised Clinical Experience, Clinical Fellowship, Official ETS-Praxis Series Scores. You will need to contact ASHA to request an original letter of verification indicating completion of all requirements to be sent directly to our office. The verification must have a current expiration date.
    10. Jurisprudence Examination: All Applicants for licensure as a speech-language pathologist or audiologist must pass a written jurisprudence examination. When your application has been approved, you will receive the jurisprudence examination and instructions. The jurisprudence examination will cover the Speech-Language Pathology and Audiology Statute (IC § 25-35), Administrative Rules (Title 880) and the Health Professions Standards of Practice (IC § 25-1-9) that can be found on the Board's home page. The passing score on the jurisprudence examination is seventy-five percent (75%). NO APPLICANT IS EXEMPT FROM TAKING THE LAW EXAMINATION.

    Please note that you must have a Speech Language Pathology license through our office to practice as a SLP in the State of Indiana, including all school districts.  School districts may also require you to have a license through the Department of Education in addition to a license issued by the Board.

  • Speech-Language Pathology & Audiology by Reciprocity

    Submit the following with your online application:

    1. Application Fee of $150.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Criminal Background Check.
    3. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    4. Positive Response Documentation: If you answer "Yes" to any disciplinary/background questions on the application, explain fully in a statement that includes all details. Include the violation, location, date and disposition. Submit copies of court documents for each instance to support the statement.
    5. Verification of State Licensure(s) - Current license verification is required.  The official license verification must be sent directly from the licensing authority to the Board at pla5@pla.in.gov. If licensee holds more than one license, all license verifications are required to be submitted by licensee's first renewal.
    6. Jurisprudence Examination: All Applicants for licensure as a speech-language pathologist or audiologist must pass a written jurisprudence examination. When your application has been approved, you will receive the jurisprudence examination and instructions. The jurisprudence examination will cover the Speech-Language Pathology and Audiology Statute (IC § 25-35), Administrative Rules (Title 880) and the Health Professions Standards of Practice (IC § 25-1-9) that can be found on the Board's home page.   • SLP Support Personnel Application Information

    Submit the following with your online application:

    1. Application Fee of $50.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Criminal Background Check
    3. Additional Documentation: Documentation may be uploaded as part of the online application process.
      • For Support Personnel:  
        • Form A-1 to be completed by the field supervisor.
        • Form A-2 to be completd by applicant's previous or current supervisor.
        • Form SLP-1 to be completed by the field supervisor.
      • Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
      • Positive Response Documentation: If you answer "Yes" to any disciplinary/background questions on the application, explain fully in a statement that includes all details. Include the violation, location, date and disposition. Submit copies of court documents for each instance to support the statement.
    4. Official Transcripts: See education requirements below. Applicants must submit an official transcript of grades, verifying that the applicant possesses an associate’s or bachelor’s degree from an accredited academic institution in the area for which the applicant is applying for licensure.
    5. Supervisor:  In order to supervise speech-language pathology support personnel, the supervisor must hold a current Indiana license as a speech-language pathologist issued by the Indiana Speech-Language Pathology and Audiology Board OR the following:
      • If before September 1, 1990, completed all the course work and obtained all the experience required to receive a life license from the department of education; and
      • Was issued a life license by the department of education;
      • Holds a certificate of clinical competence in speech-language pathology or its equivalent issued by a nationally recognized association for speech-language and hearing.

    Attention Support Personnel Applicants

    A Speech-Language Pathologist must meet the following qualifications to supervise speech-language pathology support personnel:

    1. hold a current license as a Speech-Language Pathologist issued by the board;

    2. hold ONE of the following:

    SLP Support Personnel has a Three-Tiered System Based on Level of Education:

    Speech-Language Pathology Aides
    The minimum education requirement for a SLP aide is a high school degree or equivalent.

    (1) The supervisor of the SLP aide shall provide direct supervision a minimum of twenty percent (20%) weekly for the first ninety (90) days of work and ten percent (10%) weekly thereafter. The supervisor must:

    (A) be physically present within the same building as the SLP aide whenever direct client care is provided; and

    (B) directly provide a minimum of thirty-three percent (33%) of the patient's or client's treatment weekly.


    Speech-Language Pathology Associates
    The minimum education requirement for a SLP Associate is an associate degree or its equivalent from an accredited institution in the area of speech-language pathology.  The degree program must include at least 60 credit hours with the following coursework requirements:

    1. 24 credit hours in technical content course work related speech-language pathology.  The core technical skills must include the following:

    • Instruction about normal processes of communication.
    • Instruction targeting the practices and methods of service delivery that are specific to SLP associates.
    • Instruction regarding the treatment of communication disorders.
    • Instruction targeting the following workplace behavior and skills:
      • Working with clients or patients in a supportive manner,
      • Following supervisor’s instructions.
      • Maintaining confidentially
      • Communicating with oral and written forms.
      • Following established health and safety precautions.

    2.  Clinical observation
    3.  A minimum of one hundred (100) clock hours of supervised field experience.

    Speech-Language Pathology Assistants

    The minimum education requirement for an SLP Assistant is a bachelor’s degree or its equivalent in communication disorders from an accredited institution.

    SLP Assistants are also required to obtain the following amount of clinical experience:

    • One hundred (100) hours of a clinical practicum supervised by a SLP licensed  by the Indiana Board.

    Of the 100 hours, seventy-five (75) hours must be obtained with direct face-to-face patient/client contact in the following categories:

    • A minimum of twenty (20) hours in speech disorders.
    • A minimum of twenty (20) hours in language disorders.
    • The remaining hours may be obtained in any of the following areas:
    • Speech disorders.
    • Language disorders.
    • Hearing disorders.
  • SLP Clinical Fellowship Year (CFY)

    Submit the following with your online application:

    1. Application Fee of $50.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    3. Positive Response Documentation: If you answer "Yes" to any disciplinary/background questions on the application, explain fully in a statement that includes all details. Include the violation, location, date and disposition. Submit copies of court documents for each instance to support the statement.
    4. Education: If your clinical fellowship begins prior to the date of graduation, you must submit a letter from the school which indicates that all requirements have been completed and the date the applicant will graduate – otherwise, official conferred transcripts must be submitted directly from the university.
    5. Supervisor:  In order to supervise a clinical fellow, the supervisor must hold a current Indiana license as a speech-language pathologist or audiologist issued by the Speech-Language Pathology and Audiology Board. A clinical fellowship supervisor assumes professional responsibility for services provided by the clinical fellow under his or her supervision. Supervisors must complete and sign page 3 of the application, entitled “Clinical Fellow Supervisor's Information."  
      • Clinical fellowship supervision: Clinical fellowship supervision must entail the personal and direct involvement of the supervisor in any and all ways that will permit the clinical fellowship supervisor to monitor, improve, and evaluate the clinical fellow’s performance in professional employment.
      • Transfer of supervision: If you have a change of supervision during the eighteen (18) month period, you must file a “Transfer of Supervision” application.

    Length of Clinical Fellowship: The clinical fellowship may not exceed a maximum period of eighteen (18) consecutive months. The clinical fellowship shall consist of nine (9) months full-time employment or its equivalent. A clinical fellowship of fewer than fifteen (15) hours per week will not fulfill any part of the supervised experience requirement. Registered clinical fellows who fail to complete their requirements within the allotted eighteen months must apply for a new CFY registration and the experience hours earned during the initial eighteen month period will not count toward the requirements for licensure.

    Please note that once you have completed your CFY you must then apply for licensure as a Speech Language Pathologist through our office.  Getting your ASHA certificate does not allow you to practice as an SLP, it will however assist you in obtaining your license through our office.

Renewal Instructions

  • Renewal Information

    Renewal notices are sent approximately ninety (90) days prior to the expiration date. License holders with valid email addresses on file will be emailed the renewal notice. Those who do not have valid email addresses on file will be mailed the renewal notice; this notice is mailed to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore, when a notice has been mailed to a valid email address or mailed, the duty of the Board has been performed.

    Speech-Language Pathologists & Audiologists

    • Licenses expire December 31st ever odd year
    • Renewal Fee is $100.00

    SLP Support Personnel

    • Licenses expire December 31st annually
    • Renewal Fee is $25.00
    • Special Notes:
      • If a renewal is not submitted by expiration, you must apply for a new license.
      • If you are employed at the same facility/school as you were under your previous supervisor and have just changed supervisors you need to have your new supervisor complete the SLP-1 Form. This can be submitted now electronically at MyLicense One by logging into your account.
      • If you have changed employers since you last renewed you cannot renew your existing license, but must apply for a new license by completing the SLP Support Personnel application and all the requirements.
  • Reinstatement Information

    Expired Three (3) Years or more

    If your SLP or AUD license has been expired for three or more years, you must reinstate your license to practice.  Please submit your reinstatement online with the additional documentation:

    • Payment of the current renewal fee ($100) and current initial application fee ($150).  Total reinstatement fee:  $250.
    • Documentation to Upload:
      • Copies of certificate of completion for continuing education: Expired 3-6 years requires 36 Hours; Expire 6+ years 72 Hours with at least 36 completed in the last 2 years.
      • Letter of work history or resume detailing any work since your license has expired. If unemployed, current ASHA registration.
      • Verification of any other state license held.
      • Indiana Law Exam: This will be sent to you once you have submitted the additional documents

    *Support Personnel Licenses are not reinstateable.  Go to SLP Support Personnel Application Information.

  • Continuing Education

    Speech-language pathologists and audiologists are required to have completed thirty-six (36) hours of continuing education for each two-year renewal cycle. Continuing education clock hours must be obtained within the biennial renewal period and may not be carried over from one renewal cycle to another.   If you are licensed for less than a year you are not required to have any CE's, but if you have been licensed for more than a year you must have the full 36 hours. Please note that in-service and competency CE's cannot be counted toward your renewal CE.  Also, you can only count a CPR CE the first time you renew your license.

    Continuing Education Activities

    The continuing education requirement may be satisfied by participating in the following activities:

    • Educational programs presented by state scientific and professional organizations.
    • University courses taken beyond those required for initial licensure if the course content pertains to the practice of speech-language pathology or audiology.
    • Lectures or workshops, live or recorded telecast, if the educational program is relevant to the practice of speech-language pathology or audiology.

    Self-Study Continuing Education

    Self-study shall include the following:

    • journals and books;
    • audio tapes
    • films and video tapes;
    • visits to facilities;
    • meetings with other professionals and
    • writing articles for publication.

    However, the learning experiences derived from the above must pertain to the practice of speech-language pathology or audiology and no more than six (6) hours of the continuing education requirement may be acquired though self-study.

    Approved Organizations

    Under Ind. Code 25-1-4-0.2, if a program is designed to directly enhance the  knowledge and skill in providing services relevant to the profession or occupation and falls under an “Approved Organization,” it is not necessary to submit an application for approval by the Board.

    Ind. Code 25-1-4-0.2:
    Sec. 0.2. As used in this chapter, “approved organization” refers to the following:

    • (1) United States Department of Education.
    • (2) Council on Post-Secondary Education.
    • (3) Joint Commission on Accreditation of Hospitals.
    • (4) Joint Commission on Healthcare Organizations.
    • (5) Federal, state, and local government agencies.
    • (6) A college or other teaching institution accredited by the United States Department of Education or the Council on Post-Secondary Education.
    • (7) A national organization of practitioners whose members practicing in Indiana are subject to regulation by a board or agency regulating a profession or occupation under this title or IC 15.
    • (8) A national, state, district, or local organization that operates as an affiliated entity under the approval of any organization listed in subdivisions (1) and (7).
    • (9) An internship or a residency program conducted in a hospital that has been approved by an organization listed in subdivisions (1) through (7).
    • (10) Any other organization or individual approved by the Board.
