Occupational Therapy Licensing Information

Application Instructions

  • General Information

    The Fair Information Practice Act:  In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications:  If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • OT/OTA Application by Examination

    Submit the following with your online application:

    1. Application fee of $100.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Criminal Background Check
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement.
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    5. Transcript: A transcript of grades from the school from which the occupational therapist or occupational therapy assistant obtained his or her degree that shows the degree conferred. Graduates of a foreign occupational therapy program must submit an official translation if the transcript is not in English.
    6. Diploma: Certified copy of Diploma or an original letter from the school indicating the applicant has completed the degree and has graduated (graduation date must be listed) and this letter must be signed by the program director/dean of program. The letter must have an original signature or the school seal if it’s a stamped signature.
    7. Exam Score Report: Score report submitted directly to the Committee from NBCOT.
    8. Verification of State Licensure: If you hold a license or certification in another State, you must provide verification of that information. Verifications must be completed from the state of issuance and sent to the Indiana Occupational Therapy Committee.  Those jurisdictions may provide their own verification, or complete our verification form. Copies of license cards from other jurisdictions are not accepted as proof of verification of licensure.

  • OT/OTA Application by Reciprocity

    Submit the following with your online application:

    1. Application fee of $100.00: Pay by credit or debit card. All application fees are nonrefundable.

Copies of license cards from other jurisdictions are not accepted as proof of verification of licensure. In order to issue your Indiana license by Reciprocity, you must have at least 1 license verification submitted.*

    *If you are licensed in multiple states and obtain your Indiana license via reciprocity, at your next Indiana license renewal, you must have the remaining license verifications on file in order to complete your renewal.  Any additional license verifications must be received by the Indiana Occupational Therapy Committee in order to renew your Indiana OT/OTA license that was obtained by Reciprocity. Failure to submit the remaining license verifications will result in an incomplete renewal and your Indiana OT/OTA license may expire and remain expired until the verifications are received.

  • Temporary Permits

    Submit the following with your online application AFTER you have submitted your application for full licensure:

    1. Application fee of $50.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Affirmation of Supervision: You must upload your completed Affirmation of Supervision section found on page 3 of State Form 43826.
    3. A person with a temporary permit may only practice occupational therapy or occupational therapy assisting under the direct supervision of an Indiana certified occupational therapist that is responsible for the patient.

      A temporary permit expires the earlier of the date the person holding the permit is issued a permanent license, the date the committee disapproves the person's license application; or one hundred eighty (180) days after the date the permit is issued.

      The committee may renew a temporary permit if the person holding the permit was scheduled to take the next examination and the person if they  did not take the examination and they show good cause for not taking the examination.

  • Occupational Therapy Licensure Compact

    The Occupational Therapy Licensure Compact (OT Compact) was adopted by the State of Indiana and enacted into law by SEA 73-2023. SEA 73-2023 is effective July 1, 2023.

    The OT Compact allows eligible licensed occupational therapists and occupational therapy assistants a way for providing services in compact member states. If you are interested in learning more about the OT Compact, including information about meetings, rulemaking, and any rules open for public comment, please visit the following link: otcompact.gov

    NOTICE: The Indiana Professional Licensing Agency and the Indiana Occupational Therapy Committee will work with the Occupational Therapy Compact Commission to implement the OT Compact in the state of Indiana. Compact privileges are not available at this time. Additional information and updates will be provided as they become available.

  • Occupational Therapy Continuing Education Provider

    Individuals or organizations applying for approval as sponsors of category I continuing competency courses must submit an application for approval at least thirty (30) days prior to the date of the individual's or organization's presentation of a program for continuing competency credit.

    Submit the following with your online application:

    1. Mission Statement
    2. Instructor Resumes: A vita or resume listing all educational and relevant work experience.
    3. Brochure of Programs Offered: 
      • Organizations must include a description of the specific purposes for which the organization was formed, and individuals in the organization with direct responsibility for the training activities of the organization, a vita or resume listing all educational and relevant work experience.
      • A list of each educational program presented or sponsored by the individual or organization for one (1) year prior to the date of the request for approval.
      • For each program listed under subdivision (5), the following must be included: the date and location of the program; a brief summary of the content of the program; the name and the academic and professional background of the lecturer; the number of clock hours of continuing education credit granted by a state licensing or similar regulatory authority for the program.
    4. Sample Course
    5. Evaluation Form: A description of the course evaluation technique utilized for all educational programs.
    6. Sample CE Certificate
    7. Number of hours requested
    8. A description of the procedure to be utilized for monitoring and documenting attendance.
      • The individual or organization is responsible for monitoring attendance in such a way that verification of attendance throughout the program can be reliably assured.

    Approval of the individual or organization will be valid for a maximum period of two (2) years. The individual or organization is responsible for applying to the committee for approval.

Renewal Instructions

  • Renewal Information
    Renewal notices are sent approximately ninety (90) days prior to the expiration date. License holders with valid email addresses on file will be emailed the renewal notice. Those who do not have valid email addresses on file will be mailed the renewal notice; this notice is mailed to the address of record with the Committee. The Committee has no way of knowing whether or not a notice reaches its destination; therefore, when a notice has been mailed to a valid email address or mailed, the duty of the Committee has been performed.

    Occupational Therapists and Occupational Therapy Assistants

    • Occupational Therapist and Occupational Therapy Assistant licenses expire December 31 of even-numbered years.
    • The renewal fee for Occupational Therapists and Occupational Therapy Assistants is $100.
    • Occupational Therapist and Occupational Therapy Assistants are required to obtain eighteen (18) hours of continuing competency prior to renewal. For more information regarding Continuing Competency please see the next section.

    Continuing Education Sponsor

    • Occupational Therapy Continuing Competency/Education Sponsors expire November 30th annually.
    • To renew online, you must ensure your license is linked on your MyLicense One dashboard, and your renewal will include your License Number and Registration Code.  If you cannot locate your Registration Code, please contact pla14@pla.in.gov.
    • There is no renewal fee.
    • Submit the following with your online renewal:

  • Reinstatement Information

    Occupational Therapists and Occupational Therapy Assistants

    If your license has been expired for three or more years, you must reinstate your license to practice.  Please submit your reinstatement online with the additional documentation:

    • Payment of the current renewal fee ($100) and current initial application fee ($100) for a total reinstatement fee:  $200.
    • Documentation to Upload:
      • Copies of Continuing Competency certificates of completion completed since your license expired
      • Letter of Work history detailing your work since your license expired
      • Verification for any and all licenses ever held
    • Personal appearance before the Committee may be schedule (they may request you retake the examination)
  • Continuing Education Requirements

    All active licensees who were valid for less than twelve (12) months, no continuing competency is required for renewal.  If the license is valid for twelve (12) to twenty-four (24) months, nine (9) hours of continuing competency is required for renewal. If the license was valid for twenty-four (24) months, eighteen (18) hours of continuing competency is required for renewal. Nine (9) hours must be Category I courses; not more than nine (9) hours may be earned through Category II credits.  IF YOU HAVE NOT COMPLETED YOUR CC – do not renew your license until you have met the requirements.

    Continuing Education Requirements

    Eighteen (18) hours of continuing competency are required for each license renewal.

    Programs MUST be approved by the Committee or be sponsored by one of the following:

    1. American Occupational Therapy Association or AOTA approved providers.
    2. National Board for Certification in Occupational Therapy.
    3. Indiana Occupational Therapy Association or IOTA approved providers.

    Continuing competency hours must be obtained within the biennial renewal period; and may not be carried over from one (1) licensure period to another.

    If a license or certificate is valid for less than twelve (12) months, no continuing competency is required for renewal. If the license or certificate is valid for twelve (12) to twenty-four (24) months, nine (9) hours of continuing competency is required for renewal.

    Audits for compliance with continuing competency requirements and actions regarding noncompliance will be conducted as provided for in IC 25-1-4.

    A license or certificate holder must do the following:

    1. Certify completion of continuing competency required by this rule at the time of license renewal.
    2. Retain verification of completion of continuing competency required by this rule for three (3) years after the last renewal date.
    3. Present verification of completion of continuing competency required by this rule at the request of the committee.

    During each two (2) year license period, an occupational therapist or occupational therapy assistant must complete at least eighteen (18) hours of continuing competency of which at least nine (9) hours must be in Category I courses.

    An occupational therapist or occupational therapy assistant may not earn more than nine (9) Category II credit hours towards the requirements under this section.

    All eighteen (18) hours of the continuing competency requirements may be obtained through distance learning methods provided it is obtained from an approved provider.

    Category I continuing competency courses include the following and must be at least one (1) contact hour in length:

    1. Formally organized courses.
    2. Workshops.
    3. Seminars.
    4. Symposia.
    5. Home study programs, including approved online or web-based, audio, and video instructional programs.
    6. Undergraduate or graduate courses from an accredited institution related to the management, practice, and education of occupational therapy.

    Category II continuing competency activities include the following:

    1. Publications of books, articles, or films related to practice, management, or education of occupational therapy. A maximum of five (5) contact hours will be awarded under this subdivision.
    2. Preparation for teaching a course in occupational therapy in an accredited program. A maximum of six (6) contact hours will be awarded under this subdivision.
    3. Supervision of fieldwork for Level I and Level II students. One (1) contact hour will be awarded for every thirty-two (32) hours of supervision with a maximum of six (6) contact hours.
    4. In-house or in-service seminars specifically designed for training or teaching.
    5. Actively participating with professional organizations as an officer or committee chairperson.
    6. Supervising occupational therapy students when doing their observational hours as a pre-OT student applicant.
    7. Research activities other than publications and presentations.

    Continuing education courses offered by accredited colleges are acceptable for purposes of this rule if the course content is designed to directly enhance the practitioner's knowledge and skill in providing services relevant to the practice of occupational therapy.

    The following conversion will be used for this type of credit:

    1. One (1) semester hour equals fifteen (15) contact hours.
    2. One (1) quarter hour equals ten (10) contact hours.

Fee Schedule
