Dietitians Licensing Information

Application Instructions

  • General Information

    The Fair Information Practice Act:  In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications:  If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • Dietitian by Examination

    Submit the following with your online application:

    1. Application Fee: $20 application fee to be paid by credit or debit card.  All application fees are nonrefundable.
    2. Criminal Background Check
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, provide a detailed statement that includes all relevant details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    5. Official Transcript - Submit official transcripts from your college and/or university from where you received your degree.
    6. CDR Card - If you are registered with the Commission on Dietetic Registration (CDR), please submit a copy of your current registration card that indicates the period of registration and the expiration date.

    Examination
    The Commission on Dietetic Registration (CDR) has contracted with the American College Testing Program (ACT) to administer the Registration Examination for Dietitians. The examination is computerized and currently given year round, Monday through Friday. Cost of the examination payable to the CDR in the form of a certified check, cashier's check or money order.  See their website for the exact cost.   • Dietitian by Credentials

    Submit the following with your online application:

    1. Application Fee: $20 application fee to be paid by credit or debit card.  All application fees are nonrefundable.
    2. Criminal Background Check
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, provide a detailed statement that includes all relevant details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    5. Official Transcript - Submit official transcripts from your college and/or university from where you received your degree.
    6. CDR Card - If you are registered with the Commission on Dietetic Registration (CDR), please submit a copy of your current registration card that indicates the period of registration and the expiration date.
    7. Verification of State Licensure(s): Verification of all registrations, licenses, or certifications to practice any health-related profession or occupation in other states or territories is required, regardless of status or expiration. Official verification must be sent directly from the licensing authority to the Board at pla3@pla.in.gov.
  • Dietitian by Reciprocity

    If you are a licensed Dietitian in another State and meet the reciprocal requirements as defined in IC 25-1-21-5, submit the following with your online application:

    1. Application Fee: $20 application fee to be paid by credit or debit card.  All application fees are nonrefundable.
    2. Criminal Background Check
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, provide a detailed statement that includes all relevant details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    5. Verification of State Licensure(s): Current license verification is required. The official verification must be sent directly from the licensing authority to the Board at pla3@pla.in.gov. If the licensee holds/held more than one license, all license verifications must be submitted by the Indiana Dietitian license's first renewal cycle.

Renewal Instructions

  • Renewal Information
    • Dietitian licenses expire on December 31 of even-numbered years.
    • The dietitian license renewal fee is $20.00 if paid before December 31. Renewals submitted after December 31 will be assessed an additional $10.00 late fee.
    • Licensed dietitians are required to complete continuing education for renewal. See below section for CE requirements.
    • Inactive renewals are now available online. Continuing education is not required for inactive renewal.

    Renewal notices are sent approximately ninety (90) days prior to the expiration date. License holders with valid e-mail addresses on file will receive the renewal notice by e-mail. Those who do not have a valid e-mail address on file will receive the license renewal notice by mail; it is sent to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore, when a notice has been e-mailed to a valid e-mail address or mailed, the duty of the Board has been performed.

    Inactive Status
    Licensee may request to place their Indiana Dietitian licenses into Inactive status by either:

    • If the license is due for renewal, submit a renewal application online, and the licensee will be given the option to renew their license into Inactive status. There's no Continuing Education requirement for renewing a license to Inactive status. OR;
    • Submit a request via email to the Board at pla3@pla.in.gov, or upload it to your license if outside the Dietitian renewal window.

    Licensee may not practice as a Dietitian without an active Indiana Dietitian license.

    Activation of an Inactive License
    A dietitian may request to activate their license from Inactive status by completing the following:

    • In MyLicense One, select the "Request Activation" action item from your linked Dietitian license.
    • Upload certificates of completion for the required Continuing Education.
    • Pay the Activation fee of $10.00

    If your Inactive Dietitian license has expired for less than 3 years, or if it's unexpired but within the Dietitian's general renewal window (90 days before expiration), you must first renew your Dietitian license to update its expiration date before submitting an Activation request using the above instructions.  If your Inactive Dietitian license has expired for 3 years or more, please see the Reinstatement Information below.

  • Reinstatement Information

    If your Dietitian license has expired for three (3) years or more, you must reinstate your license to practice. To reinstate, you must complete the following:

    1. Reinstatement Application: Reinstatement Applications may be submitted online via MyLicense One.
    2. Application Fee: Pay the current renewal fee ($20) and the current initial application fee ($20), for a total of $40, plus additional processing fees. All fees are nonrefundable.
    3. Continuing Education: Submit certificates of completion for 30 Continuing Education hours ( see the Continuing Education Requirements).
    4. Work History: Provide your work history since the expiration of your license.
    5. CDR Registration: Upload a copy of your current CDR Registration card
    6. Positive Response Documentation: If you answer "Yes" to any questions on the application, provide a detailed statement that includes all relevant details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    7. Verification of State Licensure(s): If you hold a license from another Jurisdiction, current license verification is required. The official verification must be sent directly from the licensing authority to the Board at pla3@pla.in.gov.
  • Continuing Education

    844 IAC 19-1-6  Continuing education requirements for recertification

    (a) The amount of continuing education required for renewal of certificate is determined by the date on which the applicant for renewal initially received the certificate as specified as follows:

    • (1) An applicant for renewal who initially received certification within twelve (12) months of the expiration date is not required to complete a continuing education requirement.
    • (2) An applicant for renewal who initially received certification greater than twelve (12) months but less than twenty-four (24) months prior to the expiration date must provide proof of at least fifteen (15) hours of continuing education. The fifteen (15) hours of continuing education must be obtained subsequent to the date the applicant initially received certification but prior to the expiration date.
    • (3) An applicant for renewal who initially received certification more than twenty-four (24) months before the expiration date must provide proof of at least thirty (30) hours of continuing education during the two (2) years immediately preceding renewal.

    (b) The board shall determine which activities meet the continuing education requirements and may accept those courses approved by the CDR for this purpose. Applicants will be subject to random audit and must be prepared to submit documentation of completion of approved hours if requested by the board.
