Application Instructions
Go to MyLicense.IN.gov and create or log in to your Access Indiana, single sign-on account. Complete the online application and submit payment with a credit or debit card. It is the applicant’s responsibility to follow the instructions and ensure all requirements for licensure are submitted or requested. A customer service representative will contact you with details of what additional documentation is required to complete your application. Applications and documents are processed in the order received.
- Addiction Counselor Associate (LACA)- Bachelor Level
- Completed Application: Submit online
- Application fee of $50.00: Fee is paid online during the online application process. All application fees are nonrefundable.
- Criminal Background Check
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s). Please upload at the time of application.
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree. Please upload at the time of application.
- Official Transcripts: Applicants must upload an official transcript from each school listed on Form C. The transcripts from which you obtained your degree must show that all requirements for graduation have been met and the date the degree was conferred.
- Letter of Good Standing: If you are in your last semester of your program, and wish to have approval to take your national exam, you must provide a letter of good standing from your university.
- Form C- Verification of Coursework: Applicants for an addiction counselor associate license must have a baccalaureate in addiction counseling or in a related area as determined by the board from an eligible postsecondary educational institution that meets the requirements of the board. All education criteria must be met by postsecondary educational courses. Certifications cannot be used to meet educational requirements.
- Form P- Verification of Practicum: Applicants must complete at least one (1) supervised practicum, internship, or field experience in an addiction counseling setting that requires the applicant to provide at least three hundred fifty (350) hours of addiction counseling services. The required practicum, internship, or field experience listed in this section must have been primarily in the provision of direct addiction counseling services. This includes knowledge, skill, or experience derived from direct observations of, and participation in, the practice of addiction counseling. Supervised practice, internship, or field experience means experience gained under an individual who is supervising within his or her scope of experience and training and holds an active license at the time of the supervision as:
- An addiction counselor;
- A clinical addiction counselor;
- A mental health counselor;
- A clinical social worker;
- A marriage and family therapist;
- A physician with training in psychiatric medicine;
- A psychologist;
- A clinical nurse specialist in psychiatric or mental health nursing; or
- Another state-regulated addiction counseling professional or, if the experience was gained in a state where no regulation exists, by an addictions or behavioral health professional of equivalent status;

Verifications must be completed from the state of issuance and can be uploaded to your online account. Those jurisdictions may provide their own verification, or complete our verification form here. Copies of license cards from other jurisdictions are not accepted as proof of verification of licensure.
- The top portion of this form should be completed by the applicant and sent to the appropriate state licensing board for their submission to the Indiana Professional Licensing Agency. The form may be duplicated if you hold multiple licenses. Other jurisdictions may charge a fee to verify licensure. You may wish to contact the state boards prior to your request for verification. You do not need to complete this form if you only hold licensure or certification in the State of Indiana.
- Examination Approval: Once all documentation has been reviewed and approved, an applicant who has not taken the national exam will receive an exam approval from our office. An applicant who has been approved by the Board to take the examination must take the examination within one (1) calendar year from the date of the initial Board approval. If the applicant has not taken the examination within one (1) calendar year from the date of initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. If you have already taken one of the national exams, please contact that agency to provide an official copy of your score.
- Official Score Report: Upon completion of the ADC or Level II examination; please contact your exam agency to release your scores to our office. If you fail the examination, you will receive notification from our office via email that will include instructions on how to apply to retake the examination. Please allow 7 to 10 business days for our office to process examination results once they are received.

- the work is the independent private practice of addiction counseling; and
- the work is not performed at a place that has the supervision of a qualified supervisor.

- A clinical social worker;
- A marriage and family therapist;
- A physician who has training in psychiatric medicine;
- A psychologist;
- A clinical nurse specialist in psychiatric or mental health nursing; or
- Another state-regulated addiction counseling professional or, if the experience was gained in a state where no regulation exists, by an addictions or behavioral health professional of equivalent status;
- Form S2- Verification of Supervision: Applicants must have at least two (2) years of addiction counseling supervision completed no less than 21 months and no more than 48 months. This supervision must include at least one hundred fifty (150) hours under supervision, one hundred (100) hours of which must be under individual supervision and fifty (50) hours of which must be under group supervision under a qualified supervisor. - Verification of Licensure (if applicable): If you hold a license or certification in another State, you must provide verification of that information. - Form P-Verification of Practicum: Applicants must complete one (1) supervised clinical practicum, internship, or field experience in a counseling setting, which must include a minimum of seven hundred (700) clock hours consisting of one (1) practicum of one hundred (100) hours and one (1) internship of six hundred (600) hours with at least sixty-six (66) hours of face to face supervision. This requirement may be met by a supervised practice experience that took place away from an eligible postsecondary educational institution but that is certified by an official of the eligible postsecondary educational institution as being equivalent to a clinical mental health graduate level practicum or internship program at an institution accredited by an accrediting agency approved by the United States Department of Education or the Association of Universities and Colleges of Canada.

- Form III - Clinical Coursework: This form must be completed and submitted with your application. Please list the course titles in the areas indicated below, of the graduate courses, exactly as they appear on your transcript that in your opinion, meet the requirements. If you started your MSW prior to July 1, 1997 you do not need to submit this coursework form. The completed form may be uploaded to your account.
- Official Transcripts: Applicants must upload an official transcript from each school listed on Form III. The transcripts from which you obtained your degree must show that all requirements for graduation have been met and the date the degree was conferred.
- Form I - Verification of Supervision: Applicants must submit proof of at least four (4) hours of face to face supervision per each month of experience, provided by a licensed clinical social worker (LCSW). This supervision must be completed while you were employed, and occur no less than twenty-four (24) months. Please complete the top portion of Form I and have the supervisor(s) complete the remainder of the document. This form may be duplicated if you completed the required two (2) years of supervision with more than one supervisor. Please upload these forms to your account.
- Form II - Verification of Experience: Applicants must submit proof of two (2) years of experience in the practice of clinical social work in counseling of clients in a clinical setting. If hours are obtained in Indiana they must occur under an Active LSW license or Temporary LSW Permit. The experience must be paid, full-time and supervised to qualify as experience. Part-time experience will be considered by the Board. (Note: One thousand five hundred hours (1,500) equals one (1) year of experience.) Under no circumstances can this experience requirement be met in less than twenty-four (24) months This form may be duplicated if you completed the required two (2) years of experience with more than one employer. To qualify as experience you must have been providing clinical social worker services, including assessment and evaluation of clients, in which at least fifty percent (50%) of this time consisted of providing services directly to clients. - Examination Approval: Once all documentation has been reviewed and approved, an applicant who has not taken the national exam will receive an exam approval from our office. An applicant who has been approved by the Board to take the examination must take the examination within one (1) calendar year from the date of the initial Board approval. If the applicant has not taken the examination within one (1) calendar year from the date of initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. If you have already taken the ASWB Clinical exam, please contact that agency to provide an official copy of your score.
- Official Score Report: Upon completion of the ASWB Clinical level examination, results will be released to our office the following week from when you took your exam. The Board may issue a temporary permit to practice for applicants who are applying to take their national examination and for those who submit the following:

After you have submitted your primary license application and paid your fee, please circle back and submit an application for a temporary permit.

- 90 Day Wait: Applicants must wait a period of ninety (90) days from the date of the failed examination before being approved to retake the examination. If an applicant has been granted a waiver from the national agency they must provide proof the of the waiver with their application. This may be uploaded with their application.
- NOTE: An applicant who has failed the examination three (3) times shall submit a study plan and be reviewed by the Board prior to approval for retaking the examination.
- Study Plan: Every fourth (4th) attempt of the exam must be approved by the Board before exam approval. Please submit a study plan of how you are preparing to retake the exam.
Renewal/ Reinstatement Information
- License Renewal
All licenses for the Behavior Health and Human Services Board expires April 1st on even years. The licenses once renewed will remain valid for two years with the last day of the renewal year to be April 1st. Reminders to renew your license will be sent out on or around 90 days prior to the expiration of the license. Once your have renewed your license please check that your license has renewed by either logging into your Access Indiana Account, or by using our Free Search and Verify. It is the responsibility of the licensee to ensure that their license has been completed, and they have completed all CE requirements.

Associate Renewals: Pursuant to the below statutes, associate license types may renew their license two (2) times. The board may renew associate license types for additional periods based on circumstances determined by the board. Continuing Education Categories
Category I Continuing Education: continuing education that is formal programming, which includes the following:
- Formally organized courses.
- Workshops.
- Seminars.
- Symposia.
- Institutes.
- Home study programs, including:
- computer;
- audio;
- video; and
- instructional programs, accredited by board-approved organizations.
- Courses that are relevant to the license holder’s professional skills, which are part of the curriculum of an accredited university, college, or educational institution, shall earn fifteen (15) CEUs for each academic semester hour completed, or ten (10) CEUs for each academic quarter hour completed.
- Faculty teaching a course for the first time at an accredited university, college, or educational institution shall earn one and one-half (1½) CEUs for every hour taught.
Category II Continuing Education: continuing education that is self-directed.
NOTE: You are not required to earn any Category II hours; however, if you do use Category II hours towards you CEU requirement then you may acquire no more than 20 Category II CEUs per renewal cycle. Category II CE can be considered the following:- Journal clubs, earns one (1) CEU for each hour attended.
- Office in-services, earns one (1) CEU for each hour attended.
- Case conferences that are specifically designed for training or teaching, earns one (1) CEU for each hour attended.
- Services as an instructor, presenter, or supervisor in a relevant professional seminar, workshop, or training conference earns one (1) CEU for each hour of service, but only for the initial instruction, presentation, or supervision given.
- Research and publication of research results in a recognized professional journal or book form, earns ten (10) CEUs, but may only be claimed for the initial publication of the information.
- Providing peer review of another licensee’s therapy and skills, which includes consultation, conference, and critique, earns one (1) CEU for each hour spent with the peer for this purpose.
- Services on boards and commissions and holding office in professional organizations, specifically related to the licensee’s profession, earns one (1) CEU for each hour of service

- How many continuing education hours do I need to renew my license
- 40 hours of continuing education credits are required for each renewal cycle or 20 CEUs per year.
- Must complete at least 20 hours of Category 1 per renewal cycle.
- Must complete at 2 hours of Ethics per renewal cycle (1 hour of ethics a year)
- How many hours of continuing education do I need if I have been licensed less than 24 months?
- You will need 20 hours in order to renew your license.
- How many hours of continuing education do I need if I have been licensed less than 12 months?
- You do not need any continuing education in order to renew your license.
- How do I document my Category II CE?
- You can record your hours on a Word document or spreadsheet. You should include the date of the activity, the type of activity, and the amount of time you spent conducting the activity.
- Can I take all of my CEUs in one year or do they have to be split evenly throughout the renewal period?
- You can take as many CEUs as you like as long as you take at least 20 CEUs per year, a minimum of 10 of those hours must be Category I.
- Can continuing education credits transfer from one renewal period to the next?
- No. Credits can only be counted for the period in which they are taken.
- Do copies of my completed continuing education certificates need to be sent to the licensure board when I renew my license?
- No. Certificates are for your records and will only need to be submitted if you are audited. However, it is required that licensees maintain copies of their CEU records for a period of four years in the event they are audited by the Board.
- What do I do if I am audited?
- Once you receive your audit letter you will need to either fax or mail in copies of continuing education certificates to the Board by the deadline stated on the letter.
- What happens if I didn’t receive the necessary CEUs during a licensure cycle or can’t produce the necessary documents during an audit?
- You will be required to make up the continuing education hours you are short and you will have to pay a fine. The Board has adopted the proposed schedule as a general guideline for continuing education violations:

- Social Worker Examination
The Board has adopted the Association of Social Work Board’s (ASWB) examination to obtain a social work license. The examination is only offered in a computer format through the Association of Social Work Boards (ASWB) and its contracted examination service. The examination is offered six (6) days a week at various locations and times.
If you did not take an examination to receive licensure/certification in another state you will be required to take the ASWB examination of the appropriate licensure level before you will be licensed as a of social work in the State of Indiana.
If applicants are seeking approval for the exam, applications for the licensure must be complete prior to the Board registering applicants with examination service. Once approval is granted, information will be emailed to the applicant explaining the registration and scheduling process for the examination. It is the responsibility of the applicant to register for and schedule the examination.
An applicant who has been approved by the Board to take the examination must take the examination within one (1) year from the date of the initial Board approval. If the applicant does not take the examination within one (1) year from the date of the initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted.

Official Score Report: Upon completion of the ASWB level examination, results will be released to our office. If you passed the examination, your social work license will be issued. If you failed the examination, you will receive notification from our office via email with instructions on how to re-apply to retake the examination. Please allow 7 to 10 business days for our office to process examination results once they are received.

- Mental Health Associate & Mental Health Counselor Examination
The Board has adopted the National Board for Certified Counselors (NBCC) examinations. The Board will accept the National Counselor Examination (NCE) to obtain the mental health associate counselor license and the National Clinical Mental Health Counselor Examination (NCMHCE) to obtain a mental health counselor license. If you are currently licensed as a mental health counselor in another state your are not required to sit for the exam provided you took the NCMHCE examination in another state or an equivalent state constructed examination at the same level or higher. The National Counselor Examination (NCE) is not an acceptable examination for mental health counselor licensure.
If you did not take an examination to receive licensure/certification in the another state at the same level or higher level, you will be required to take the NCMHCE before you will be licensed as a mental health counselor in the State of Indiana.
An applicant who has been approved by the Board to take the examination must take the examination within one (1) calendar year from the date of the initial Board approval. If the applicant has not taken the examination within one (1) calendar year from the date of initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. No extensions will be granted.

This checklist is for applicants who are applying to become a Category I CE Sponsor in the State of Indiana.

- Application fee of $50.00: Fee is paid online during the online application process. All application fees are nonrefundable.
- Required Documentation: Be prepared to provide the followiing with your online application:
- Mission Statement: Applicants must provide a brief statement of their goals of the type of CE they will be presenting for the profession.
- Presenter Information: Application must provide a list of names and credentials of all presenters. Please submit a copy of their Curriculum Vitae.
- Programs Offered: Applicants must provide a list or brochure of the courses that will be offered as a CE Provider. This must include the names of the courses and where they will be held. If they are being held virtual, it must be indicated and if the course will have an exam at the end of the course.
- Sample Course: Please provide a copy sample of one of the courses that will be presented, or that you have presented previously.
- Sample Course Evaluation: Please provide a copy of the evaluation form you will be providing to professionals taking your course.
- Sample Sign-in Sheet: Please provide a sample of the sign in sheet you will be using to keep track of the professionals attending your course(s). Your sheet at minimum should contain the following:
- Name of the Program
- Name of Presenter(s)
- Date
- Location
- Sample Certificate of Completion: Please provide a copy of your certificate of completion you will provide the professionals who have completed your course(s). Your certificate should contain at minimum the number of hours awarded, completion date, and group(s) approving Presentation (e.g., ASWB, NBCC, AMFTRB).
- Record Keeping: Please provide a brief summary of how you are securing the records of those who are attending your course(s).

- What is the difference between LAC and LCAC?
- An LAC is a Licensed Addiction Counselor, which is a Bachelor education level licensure. An LCAC is a Licensed Clinical Addiction Counselor, which is a Masters education level licensure.
- What is an Associate license?
- An Associate Addiction license is for individuals to obtain post degree experience and supervision. Associate licenses may only be renewed twice. Please review the statute and rules here.
- Can I get a temporary addictions license?
- You may apply for a Temporary Addictions Permit when you apply for approval to take the exam. Your Temporary Permit will last one year, and cannot be renewed.
- What is the difference between forms E2 and S2?
- Form E2 verifies your post-degree experience and employment. Form S2 verifies your post-degree supervision while you were employed. These forms must have matching time frames.
- Can I get an addictions license if I have lots of experience working in the field?
- You may apply for a license and complete all documents for licensure. As of July 1, 2014 if you have not taken a national exam you must apply to take the exam.
- Can I use my CADAC credentials to apply by reciprocity?
- To apply by reciprocity, you must hold a license or certificate in another state and have passed an equivalent national exam. If you only hold the CADAC credentials and not a license in another state, you will not qualify for reciprocity.
- Can I use my CADAC training to meet the educational needs?
- You must show that you have completed your educational requirements with your transcripts. You cannot use certificate training or Continuing Education seminars to meet the requirements.
- What do I need to do if I would like to perform telehealth as an Addiction Counselor or Clinical Addiction Counselor?
