Application Instructions
- Licensed Behavior Analyst via Application
For Applicants who are applying as a Behavioral Analyst and who are certified as Board Certified Behavior Analyst® (BCBA®) by the Behavior Analyst Certification Board (BACB) in good standing with the Behavior Analyst Certification Board (BACB).
- Completed Application: Submit online
- Application fee of $100.00: Fee is paid online during the online application process. All application fees are nonrefundable.
- Criminal Background Check
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
The Fair Information Practice Act: In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.
Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). - Licensed Behavior Analyst via Reciprocity
For Applicants who are applying as a Licensed Behavioral Analyst by Reciprocity and hold a current license in a state or territory of the United States that requires similar education, experience, and certification as required for corresponding licensure in Indiana.
- Completed Application: Submit online
- Application fee of $75.00: Fee is paid online during the online application process. All application fees are nonrefundable.
- Criminal Background Check

- Statement of the Scope of Practice in the Other State(s) of Licensure: Provide information regarding the scope of your practice in the other state(s) where you currently hold a license. Please upload at the time of application.
- Verification of Licensure (if applicable): If you hold a license or certification in another State, you must provide verification of that information. Verifications must be completed from the state of issuance and can be uploaded to your online account. Those jurisdictions may provide their own verification, or complete our verification form here. Copies of license cards from other jurisdictions are not accepted as proof of verification of licensure.
- The top portion of this form should be completed by the applicant and sent to the appropriate state licensing board for their submission to the Indiana Professional Licensing Agency. The form may be duplicated if you hold multiple licenses. Other jurisdictions may charge a fee to verify licensure. You may wish to contact the state boards prior to your request for verification. You do not need to complete this form if you only hold licensure or certification in the State of Indiana.

- List of Programs:
- The date and location, or format, of the CE.
- A brief summary of the content of the CE.
- The name and academic and professional background of the lecturer.
- The number of hours of CE credit granted by a state licensing agency or similar regulatory authority for the program.
- A description of the course evaluation technique used for each program.
- Sample Certificate of Completion: Please provide a copy of your certificate of completion you will provide the professionals who have completed your course(s). Your certificate should contain at minimum the number of hours awarded, completion date, and group(s) approving Presentation.
- List of Instructors
- Please include a curriculum vitae or résumé listing the educational and relevant work experience for each individual in the organization with direct responsibility for the training activities of the organization,
- Attendance Procedure: A description of the procedure to be used for monitoring and documenting attendance. The organization is responsible for monitoring attendance so that verifying attendance throughout the program can be reliably assured.

Renewal Instructions
- Renewals for LBA and LABA
All LBAs and LABAs expire December 31st of the odd years. The licenses once renewed will remain valid for two years with the last day of the renewal year to be December 31st. Reminders to renew your license will be sent out on or around 90 days prior to the expiration of the license. Once your have renewed your license please check that your license has renewed by either logging into your account, or by using our Free Search and Verify. It is the responsibility of the licensee to ensure that their license renewal has been completed, they have completed all CE requirements, and attest to holding a current BCBA.
LABAs must have a signed contract with an licensed behavior analyst in order to provide ongoing supervision as specified in 844 IAC 21-4-6. The LABA is required to submit a written record of the supervision provided by LBA during the preceding renewal period. LABAs must also ensure that their supervisor license is current at the time of renewal.
- Renewal for CE Provider
Approved CE Providers are valid for two years. The licenses once renewed will remain valid for two years with the last day of the renewal year to be December 31st. Reminders to renew your license will be sent out on or around 90 days prior to the expiration of the license. Once your have renewed your license please check that your license has renewed by either logging into your account, or by using our Free Search and Verify.
- Continuing Education Requirements
In accordance with 844 IAC 21-3-1, each licensee shall complete the following continuing education units:
- (1) one (1) continuing education (CE) unit in trauma informed care; and
- (2) one (1) CE unit in either of the following:
- (A) Abuse and neglect.
- (B) Mandatory reporting.
