License History Verification Information Requests:

If you are requesting information on behalf of another Gaming Jurisdiction/Regulator, including Tribal Gaming, for an occupational license or Gaming Entity license that is not listed on the above webpages, email LicenseVerifications@igc.in.gov , including the following information for each request:

Occupational License Verification

full legal name of the Individual
date of birth
last four of their US social security number, if applicable
signed Release of Information

Gaming Entity License Verification

full legal name of the Gaming Entity
signed Release of Information for the Gaming Entity.

All other requests regarding occupational license history with the Indiana Gaming Commission or Gaming Entity license history that is not listed on the above webpages, email LicenseVerifications@igc.in.gov, including the following information for each request:
