Documentary Evidence – Amendments/Corrections List

Listed below are documents that can be used as documentary evidence to amend/correct an Indiana birth record.  These acceptable documents must be ten years old or more.  Photocopies are acceptable if they legible copies.  If acceptable evidence cannot be provided, a court order is required to amendment birth record.

  1. Baptismal certificate (may be obtained from the church)
  2. School records (Entry into grade school is the most valuable example – these may be obtained from the County Superintendent or School Office).  Records must arrive from the school sealed.
  3. Life insurance policies (The application page, usually located inside the original policy.  The name of the insurance company, policy number, and date issued must be included for this option to be valid).
  4. Application for Marriage License (available from the county office where the license was originally issued).
  5. Voter’s registration card (may be obtained from the county clerk’s office in the county where the voter is registered).
  6. Military discharge form (DD214).
  7. Census record (copies of census records must be obtained from the Bureau of Census.  Records of past censuses are available from 1880 to 1960 (with the exception of 1890).
  8. Employment Records (available from the employer’s personnel office.  Records should show the name of the place of employment and date the record was created).
  9. Records from a Professional Licensing Agency (usually available from the secretary of the organization).
  10. Birth record(s) of children (The original record of birth is normally filed with the state department of health’s Vital Records Division {if the child was born in Indiana} and will list the parent’s age at time of birth.  The state staff member will check one child’s birth record when supplied the name, place, date of birth, and name of parents).
  11. Hospital records of birth (may be obtained from the medical records office in the hospital where the delivery occurred).
  12. High school diploma
  13. Other examples of acceptable documents: Medical shot record, college diploma (or transcripts), Federal tax statements, mortgage deeds, or last will and testament.

NON - ACCEPTABLE EVIDENCE:

  • Death certificates
  • Marriage licenses
  • Driver's licenses
  • State IDs

Financial papers that are NOT Federal Tax Papers

  • Utility bills
  • Bank statements
  • Credit card statements
  • Vehicle registrations
