Health: CDPC: IN-WISEWOMAN Eligibility

To be enrolled in the IN-WISEWOMAN Program, you must meet the following requirements:

Female

Currently enrolled and remain eligible to participate in the Indiana Breast and Cervical Cancer Program (IN-BCCP)

In the priority age range of 35-64 years old

Income at or below 200% of the federal poverty line;

Underinsured or uninsured, and low income

Unable to pay the premium to enroll in Medicare or only enrolled In Medicare Part A
