To order and retain an Oversize/Overweight (OSW) permit in Indiana, you must set up an OSW Transporting Account, order your permits, and follow the general provisions of the permit.
Step 1: Set up an OSW Transporting Account
You must have an Oversize/Overweight Transporting Account set up with Indiana to obtain permits. DOR offers two ways that you can set up an account:
- Go to Indiana Motor Carrier Applications and select “Apply for OSW Account.” Be sure to have your federal identification number (FEIN), or your Social Security number if you are a sole proprietor and have no FEIN. You must also know your USDOT number (if applicable).
If you intend to apply for permits online, you must have a user ID and password. After you have established an Indiana account, you can register as a user. Go to Indiana Motor Carrier Applications and select “User Registration.”
- Complete Form M-203. If you are a permitting service, you will need Form M-201. You will need to provide your FEIN and USDOT number (if applicable). Complete and mail to:

Step 2: Order Permits
Once you have an Indiana account created and a user ID and password, you are ready to order your permit. You have two options when ordering permits:
- Go to Indiana Motor Carrier Applications and select “Login to OSW.”
- Complete Form M-233 and mail or bring to:

If you are a permitting service, you must obtain your client’s FEIN and USDOT number to order a permit on their behalf. MCS Customer Service representatives cannot process payments for permits separate from the application. Payment must accompany mailed permit applications, or the permitting service must login to the permitting system to make a payment. In-person service is available at DOR’s Motor Carrier Customer Service Center. MCS accepts Visa, Mastercard, Discover Card, and checks. Escrow accounts may be established. Step 3: Follow General Provisions
You must follow all the general provisions found on Form M-204 at all times when operating a vehicle with an Indiana Oversize/Overweight Vehicle Permit.
