Upload Additional Documentation
This service is to be used to upload additional documents for applications and licenses.
To upload documents, one must first log into their online record.
- Click "Get Started".
- Login to your Access Indiana account. (or register for one if you have not yet created one)
- Enter your First/Last Name: This entry is permanent and cannot be changed.
- Select "Don't see your license? Click here to search for it."
- You will need your complete License Number and Registration Code: If you do not know your registration code, you may contact our office (317) 232-2960.
- After linking your license, select the three dots beside your license and select the action:
- Upload additional documentation
PLA Online Services is currently not aware of any issues. In order to begin, please select Get Started >> in the section above.
