The FY 2008 Indiana Institutional Library and Media Center Survey Form (July 2007 – June 2008) from the Indiana State Library, Library Development Office is now posted online. Please visit this link to enter your data:

Completion of this survey is required for state institutions to access state-funded services through the Indiana State Library, which includes consulting services, interlibrary loan, statewide delivery and eligibility for LSTA grants. Failure to complete and return the survey will jeopardize your institution’s receipt of these services.

Please read the glossary/instructions and questions before completing the survey. Read the survey form and definitions carefully.

If your institution provides any type of educational or learning media for the institution’s offenders, residents, staff, or students, please complete and submit this survey by September 12, 2008. If your institution applies for and receives grants from the Indiana State Library, it is imperative that library-use records are kept in order to assess performance accountability.

The State Library will use the data collected from this survey for institutional library long-range planning. Your library’s statistics, with other Indiana institutional libraries, are compiled into a computerized database to produce an institutional library report that creates an accurate, complete picture of the overall status of institutional library service in Indiana.
