- The reflective material is placed anywhere other than on the front and rear, or both, of the bus between the warning lamps; each side beltline, seat line, or floor line or all three (3); outlining each required emergency exit; or across the rear of the bus directly above the rear bumper.
- There is no brake certification form the certification is expired or is not signed within the six-month requirement
- The seat belt cutter does not have a full width handgrip, with a protective blade to minimize risk
- There are any holes in the floor
