• There is no brake certification, the certification has expired, or is not signed within the six month requirement.

• Prohibited area sign is missing or obscured.
• Prohibited area line is missing or improper.

• Required reflective tape is applied in the wrong location, missing, and wrong width or badly faded
• If required, positive door opening device is missing or fails to operate properly.

• There is no brake certification, the certification has expired, or is not signed within the six month requirement.
