Welcome to the home page for the Indiana State Department of Health (ISDH) Division of Health Care Education and Quality.  The Division is responsible for the licensing, certification, or registration of certified nurse aides, home health aides, and qualified medication aides.  The Division approves aide education programs.  The Division also coordinates the training of state health care facility surveyors and oversees the processing of survey reports.

The Indiana State Department of Health (ISDH) invites you to attend the Indiana Long Term Care Leadership Conference, October 22, 2015 in Indianapolis. The ISDH is planning an outstanding program focusing on “Expressive Arts.” The ISDH invites you to join us as we address this important healthcare topic.

The ISDH invites each comprehensive care facility (nursing home) to send two representatives to the conference. You should consider sending your activity director to this conference. Each facility may select the individuals attending the conference on behalf of their facility. The ISDH recommends individuals such as the facility administrator, director of nursing, or compliance officer. Corporate offices of facilities are also invited to send appropriate representatives.

The ISDH will also be inviting selected professional organizations and individuals to attend the conference. Invited organizations will include the Alzheimer's Association of Greater Indiana, QSource, Hoosier Owners and Providers for the Elderly, Indiana Health Care Association, Indiana Hospital Association, Leading Age Indiana, Indiana Ombudsman, patient safety coalitions and centers, regional collaborative, and United Senior Action. The ISDH will also be inviting academic and legislative partners. ISDH long term care surveyors will be attending.

Report a Complaint or Incident to the ISDH

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Report Form is for health care facilities to notify the Indiana State Department of Health of a reportable incident pursuant to the ISDH Reportable Unusual Occurrence Policy.  The Incident Report Form is also for health care facility staff (nursing homes, intermediate care facilities, and hospice agencies) to report a reasonable suspicion of a crime against a resident pursuant to Federal regulations.  This form is not to be used to file a complaint.
