What is it and why is it necessary?

What is a Certificate of Service?

A certificate of service is a statement certifying that a copy of the petition has been served on or provided to the opposing party.

Why is it necessary?

Indiana law states that in order to obtain a review by the Indiana Board a party must (1) file a petition with the Indiana Board; and (2) mail a copy of the petition to the other party

What does the Certificate of Service do?

The Certificate of Service states the date the petition was served, the name and address of the person served, and the signature of the person serving the petition.  By completing the Certificate of Service you affirm under penalties of perjury that you have served a copy of the petition to the other party.

Where do I find the Certificate of Service?

The Indiana Board has included the Certificate of Service as Section 6 of the Form 131 and Form 132 petitions.
