Facility Profile Reports

The Indiana State Department of Health is providing a detailed profile of all certified and non-certified comprehensive care nursing facilities within the state of Indiana. These profiles provide information on the following; 1) Basic information, 2) Bed counts and census, 3) Results of surveys, 4) State licensure actions, and 5) Federal certification actions imposed. You may search for facilities by knowing the city or county in which it’s located.

In addition to the information provided on this web site, the Centers for Medicare & Medicaid Services (CMS) has a web site which contains information on every Medicare and Medicaid certified nursing home in the country. The CMS nursing home survey results contain summary information about a facility's noncompliance of regulations regarding the care of residents found in the nursing homes. You can locate nursing homes in your area and find information about compliance with federal regulations. Click here to get to the CMS nursing home compare page. Further, all information and procedures contained herein should be very carefully reviewed and should serve only as a guide for use in specific situations.

It is important to note that the facility profile pages are updated the first week of every month, and the data presented is aged approximately 1 month before being published. The aging of the data is required to insure that confidential survey information is not made public before it is legally allowed. Therefore, by the end of every month, the data contained in the facility profile pages is approximately two months old. Furthermore, please be aware that some data presented in the facility profile pages is based upon information provided by each facility at the time of their annual health survey. This data is only updated when a facility has another annual health survey, which must be between 9-15 months after the previous survey. This information will only be updated approximately once every year.

In addition to being aware of the dates and age of the data, it is important to understand that the majority of the data for every facility is manually entered into a database, which does leave the potential for human error. If an error was made by the Indiana State Department of Health (ISDH), it is immediately corrected in the database and web page upon discovery. The facilities and public are encouraged to notify the ISDH of any discrepencies in the data presented in order to improve our overall data quality. However, before contacting ISDH, please consider the age and date of the data. Comments and corrections can be e-mailed to ISDH at Providers@isdh.in.gov.
