Report a Complaint, Incident or Crime to the Indiana Department of Health
Report a complaint regarding a health care facility
Any individual may submit a complaint to the IDOH about care provided at any licensed or certified Indiana health care facility or entity. The IDOH investigates all complaints. A Complaint report form is provided for individuals to assist in the reporting of care issues.
Report an incident regarding a health care facility
State or federal regulations require health facilities or entities to report certain types of “incidents” to the IDOH. The above link is to a page that provides information about the incident reporting process and how a health care facility or entity reports incidents. An Incident report form is provided for health care facilities to report incidents to the IDOH.
Report a reasonable suspicion of a crime against a long term care resident
Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long term care resident to the IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include a nursing home, hospice, or intermediate care facility for individuals with intellectual disabilities (ICF-IID).
