Governor Mitch Daniels recently began sending special certificates to the survivors of deceased Hoosier veterans honoring their service in the military. The initiative, called the Governor’s Memorial Certificate program, is the State of Indiana’s final thank you to the men and women who served in the United States Armed Forces, including the National Guard and Reserve.

Modeled after the Presidential Memorial Certificate offered by the federal government, the certificate can be obtained by contacting a person’s local funeral director, any veterans’ organization, such as the American Legion or the VFW, the County Veterans’ Service Officer or by completing the application form and contacting IDVA directly by clicking on the following link Governors Memorial Certificate. A copy of the veteran’s DD Form 214 or Record of Military Service must accompany the initial application.

The certificate, signed by Governor Daniels will be sent in a beautiful blue certificate holder containing the seal of the great State of Indiana. Additional certificates can be ordered.

Walk-In Policy Change

Due to recent policy changes at the VA Regional Office, we will no longer be able to process walk-in applications. We apologize for the inconvenience this has caused. You may still drop off applications to be processed, but please keep in mind that because of these policy changes the average wait time may take up to seven working days to process. Remission of Fees applicants should pay special attention to this time delay.
