Public Records Request Web Form
The Indiana Department of Homeland Security (IDHS) Office of General Counsel manages and maintains public records and responds to public records requests made to the agency in accordance with the Indiana Access to Public Records Act (Indiana Code 5-14-3). IDHS is committed to providing maximum transparency to the public with very few exceptions.
Prior to submitting a request using the IDHS Public Records Request Web Form below, please review the following information, which provides valuable insight into records that may be withheld from public disclosure and anticipated timelines for processing requests. Links may also be provided if records are available through a public-facing website or database without the need to submit a public records request.
Requests are received and processed as quickly as possible by IDHS. Depending on the type of record requested, complexity of the request and staffing availability, the entire process from receipt of the request, receipt of the records, review of the records and distribution to the requestor may be 4–6 weeks or longer.
- Building Plans | Construction Design Releases
Building plans are withheld from public disclosure pursuant to Ind. Code 5-14-3-4(a)(4), Ind. Code 5-14-3-4(b)(18) and Ind. Code 5-14-3-4(b)(19).
Construction design releases are publicly available at Project Activity (Includes Real-Time Plan Submissions and Releases). Code variances are publicly available at Search IDHS Variances Database.
- Fire/EMS Certifications and Licenses
IDHS offers a public-facing portal where you can request a copy of firefighter and EMS certifications and licenses. It is located in the Acadis Portal. You will need the individual's Public Safety Identification (PSID) number to search for certifications and licenses.
- Fire/EMS Incident and Run Reports
All requests for Emergency Medical Services run data should be directed to the Indiana Management Performance Hub.
- Fire Investigation Reports
Contents of fire investigation files are withheld pursuant to Ind. Code 5-14-3-4 as law enforcement records and only released through subpoena or to an insurance company pursuant to Ind. Code 27-2-13.
A "request for assistance" or "call log" are the only agency records which may be available for public disclosure.

- Personnel and Employee Records
Requests for personnel or employee records should be submitted to the Indiana State Personnel Department.
- School Safety Information
Information about the Indiana Secured School Safety Board, the Indiana School Safety Hub, the Indiana Secured School Safety Grant Program and other school safety initiatives and programs is publicly available at Secured School Safety Grant Program.
- Storage Tanks and Environmental Records
Certain information may be available through the Indiana Department of Environmental Management Virtual File Cabinet.
