Appointments and Cancellations

The Professional Licensing Agency is now accepting scheduled appointments. Please see the information below on what transactions are available and how you can schedule an appointment. If you can conduct your business with the PLA via online services, phone, or mail, please continue to do so. The PLA will follow CDC guidelines to ensure the safety of customers and employees.


The following services are available for appointment at the PLA offices:

  • Apply for a New License
  • Renew an Existing License
  • Change Personal Information
  • Compliance Inquiry

The appointment is only for 30 minutes and cannot be extended. Before arriving for your appointment, please ensure that you have all of the following prepared and completed in advance, if applicable:

  • A completed Initial Application or Renewal Form, found on the PLA website.
  • Payment, either as exact cash (no change is available) or a check or money order made to the Indiana Professional Licensing Agency.
  • Supporting Documentation, such as copies of any required licensure documentation.
  • Confirmation of your appointment, either an email printout or a digital version.

If you do not have your documents or payment ready and your appointment reaches the 30 minute mark; the appointment will end.
